摘  要:目的检测子痫前期胎盘组织中SMARCA5基因的表达,探讨其表达量与子痫前期滋养细胞侵袭、凋亡活力的内在联系。方法选取2016年2月至2018年4月在枣阳市第一人民医院分娩的91例子痫前期患者作为研究对象。根据病情严重程度分为轻度子痫前期组(n=63)、重度子痫前期组(n=28),另取同期进行分娩的100例健康产妇作为对照组。比较三组孕妇胎盘组织中SMARCA5基因表达量,以及滋养细胞相关侵袭基因、凋亡基因的表达,采用Pearson检验评估子痫前期胎盘组织中SMARCA5基因表达量与滋养细胞侵袭、凋亡活力的相关基因表达量的关系。结果三组研究对象胎盘组织中SMARCA5 mRNA表达量,滋养细胞侵袭相关基因CCL28、Gadd45α、GPR30、RASA1、HIF-2α及凋亡相关基因HtrA1、Survivin、SHBG、XIAP mRNA表达量的差异均具有统计学意义(均P<0.05)。随子痫前期病情加重,胎盘组织中SMARCA5基因表达量增加;胎盘组织中CCL28、GPR30 mRNA表达量下降,Gadd45α、RASA1、HIF-2αmRNA的表达量增加;胎盘组织中HtrA1 mRNA的表达量增加,Survivin、SHBG、XIAP mRNA的表达量下降,差异具有统计学意义(P<0.05)。相关性分析发现,子痫前期胎盘组织中SMARCA5基因表达量与滋养细胞的侵袭及凋亡相关基因表达量均直接相关,差异具有统计学意义(P<0.05)。结论子痫前期胎盘组织中SMARCA5基因表达量异常降低,且具体表达量与滋养细胞的侵袭活力及凋亡进度密切相关,对子痫前期发生发展具有重要意义。Objective To detect the expression of SMARCA5 gene in placenta of preeclampsia,and explore its correlation with the invasion and apoptosis of trophoblast cells in preeclampsia.Methods 91 preeclampsia patients delivered in our hospital from February 2016 to April 2018 were divided into mild preeclampsia group(n=63)and severe preeclampsia group(n=28)according to the severity of disease.Another 100 healthy pregnant women delivered at the same period were selected as the normal control group.The expression of SMARCA5 gene,trophoblast-related invasion genes and apoptosis genes in placenta were compared among the three groups.Pearson test was used to evaluate the correlation between SMARCA5 gene expression and the expression of invasion and apoptosis of trophoblast cells in preeclampsia.Results There were significant differences in expression of SMARCA5 mRNA,trophoblast invasion related genes CCL28,Gadd45 alpha,GPR30,RASA1,HIF-2αand trophoblast apoptosis related genes HtrA1,Survivin,SHBG,XIAP mRNA in placenta among the three groups,with statistically significant differences(all P<0.05).As aggravation of preeclampsia,the expression of SMARCA5 gene in placenta increased;the expression of CCL28 and GPR30 mRNA in placenta decreased;the expression of Gadd45 a,RASA1 and HIF-2αincreased;the expression of HtrA1 mRNA in placenta increased;the expression of Survivin,SHBG and XIAP mRNA decreased(P<0.05).Correlation analysis showed that the expression of SMARCA5 gene in placenta of preeclampsia was directly related to trophoblasts invasion and apoptosis-related genes in preeclampsia(P<0.05).Conclusions Expression of SMARCA5 gene in placenta of preeclampsia decreases abnormally,and the specific expression level is closely related to invasive activity of trophoblasts and progress of apoptosis,which is of great significance to the occurrence and development of preeclampsia.
