果酸联合黄光治疗黄褐斑患者的临床疗效及皮肤生理指标的影响

摘　　要：目的探究果酸剥脱联合黄光治疗黄褐斑的临床疗效。方法将90例确诊的黄褐斑患者随机分为试验组和对照组。试验组给予20%果酸剥脱治疗,2周/1次,每次停留3min,共4次,果酸剥脱治疗1周后,使用光动力治疗仪照射患者患处,20 min/次,1周/1次,共8周;对照组单纯应用果酸剥脱治疗。两组于治疗8周后进行黄褐斑面积及严重程度(MASI)评分及皮肤特征定量评价。结果两组患者年龄、病程、黄褐斑分型以及面积评分、颜色评分、DLQI评分等一般资料对比均为显著性差异,具有可比性;治疗1个疗程后,与治疗前相比,试验组黄褐斑面积评分、颜色评分和生活质量DLQI评分均显著下降(P<0.01);且组间对比,试验组较对照组下降更为显著;两组患者孕激素(P)水平均较治疗前明显升高,雌激素(E2)水平明显下降(P<0.05);且试验组较对照组升高或下降更为显著;试验组患者临床有效率为88.89%较对照组71.11%明显升高(P<0.05)。结论果酸剥脱联合黄光治疗黄褐斑疗效确切,且可改善患者生活质量,值得推广。Objective Exploring the clinical efficacy of acid stripping combined with yellow light in the treatment of chloasma.Method 90 patients with confirmed melasma were randomly divided into the experimental group and the control group.The experimental group was given 20%acid exfoliation treatment,2 weeks/1 time,each stay for 3 minutes,a total of 4 times.After 1 week of fruit acid exfoliation treatment,the patient was irradiated with photodynamic therapy device for 20 min/time,1 week/1 time for 8 weeks;the control group was treated with acid stripping alone.The two groups were evaluated for melasma area and severity(MASI)score and quantitative evaluation of skin characteristics after 8 weeks of treatment.Results The age,disease duration,chloasma classification,area score,color score,DLQI score and other general data were significantly different and comparable.After treatment for one course,compared with before treatment,the test group was yellow.The brown spot area score,color score and quality of life DLQI scores were significantly decreased(P<0.01);and the comparison between groups was more significant than that of the control group;the progesterone(P)levels in the two groups were significantly higher than those before treatment.Elevated,estrogen(E2)levels decreased significantly(P<0.05);and the experimental group increased or decreased significantly compared with the control group;the clinical effective rate of the experimental group was 88.89%compared with the control group 71.11%significantly(P<0.05).Conclusion Acidic acid stripping combined with yellow light is effective in treating chloasma and can improve the quality of life of patients,which is worth promoting.
