脱细胞猪真皮基质敷料在小儿特重度烧伤深Ⅱ度创面护理中的应用

摘　　要：目的探讨脱细胞猪真皮基质敷料在小儿特重度烧伤深Ⅱ度创面护理中的应用。方法选取2016年9月-2018年8月我院收治的特重度烧伤患儿40例,对深Ⅱ度创面均采用脱细胞猪真皮基质敷料,并给予治疗期间的护理干预,治疗结束后进行随访观察。观察患儿创面愈合、换药、疼痛情况,并进行伤口周围皮肤评分,随访结束后统计创面愈合质量。结果40例患儿平均手术时间为(42.36±10.59)min,均平稳度过休克期;浅Ⅱ度创面平均愈合时间为(8.36±2.15)d,平均换药次数为(2.46±0.58)次;深Ⅱ度创面平均愈合时间为(19.56±3.42)d,平均换药次数为(6.33±1.42)次;平均VAS评分(3.56±1.25)分,平均伤口周围皮肤评分(3.86±1.02)分。所有患儿随访3-10个月,其中创面愈合质量良好33例(82.50%),7例(17.5%)出现轻度瘢痕增生,均未进行植皮手术。结论小儿特重度烧伤深Ⅱ度创面应用脱细胞猪真皮基质可以促进创面愈合,治疗期间给予有效的护理干预可以有效减轻患儿的疼痛程度,促进肢端血运,加快创面愈合时间,提高创面愈合效率及质量。Objective To investigate the application of decellularized porcine dermal matrix dressing in the treatment of deepⅡdegree wounds in children with severe burns.Methods Forty children with severe burns treated in our hospital from September 2016 to August 2018 were selected.decellular porcine dermal matrix dressing was used for deep II wound.All patients were given nursing intervention during treatment.Follow-up observation was performed after treatment.The wound healing,dressing change,and pain were observed,and the skin score around the wound was evaluated.The quality of wound healing was counted after the follow-up.Results The average operation time of the 40 children was(42.36±10.59)min,and they all passed through the shock phase smoothly.The average healing time of shallowⅡdegree wounds was(8.36±2.15)days,and the average number of dressing changes was(2.46±0.58)times.The average healing time of deepⅡdegree wounds was(19.56±3.42)days,the average number of dressing changes was(6.33±1.42)times,the average VAS score was(3.56±1.25)scores,and the average skin score around the wound was(3.86±1.02)score.All patients were followed up for 3-10 months,of which 33 cases(82.50%)had good wound healing quality,and 7 cases(17.5%)had mild scar hyperplasia.No skin grafting was performed.Conclusion The application of decellularized porcine dermal matrix to deep wounds of children with severe severe burns can promote wound healing.E ffective nursing intervention during treatment can effectively reduce the degree of pain in children,promote limb blood flow,accelerate wound healing time,and Improve wound healing efficiency and quality.
