低价药品可获得性与价格变化趋势研究——基于南京市2013-2018年的数据分析

摘　　要：本文以低价药品的可获得性、价格为研究对象,基于南京市医疗机构2013-2018年的药品储备和采购数据,利用可获得性、固定拉氏价格指数指标,分层分析药价改革前后低价药品的可获得性与价格变化趋势,并运用广义定序Logit模型分析药品可获得性高低的影响因素。研究发现:近年来,低价药品可获得性总体有缓慢波动上升趋势,基本药物的可获得性高于非基本药物;2013-2015年间低价药品的价格保持平稳,2016年开始明显上涨;不同属性的低价药品对政策的响应程度不同,基本药物、医保报销比例高的、市场规模小的低价药涨幅较大。政府的系列措施确保了低价药的供应保障。建议在进一步落实短缺药品直接挂网采购的基础上,加强价格监测与管理,促进药品保供稳价。Taking availability and price of low-price drugs as the research object,based on the drug reserve and purchasing data collected from 2013 to 2018 in Nanjing City,availability rate and Laspeyres Index were used to explore the availability and price trend of drugs from different aspects before and after the reform of drug price,respectively.Generalized ordered logit model was computed to measure predictors of availability.The current study found that there was a slow upward trend in the availability of low-price drugs.The availability of essential medicines was higher than non-essential medicines.The price of low-price drugs remained stable between 2013 and 2015,and began to rise obviously from 2016.The policy response of different types of drugs was different.For essential medicines,drugs with higher reimbursement rates and drugs with smaller market,their price increases were more obvious.A series of measures taken by governments have ensured the supply of low-price drugs.It is suggested that price monitoring and management should be strengthened to promote drug supply and stabilise price on the basis of further implementation of direct online procurement of drugs in shortage.
