局部肌肉振动联合氨糖软骨素钙片对膝骨关节炎患者关节功能的影响

摘　　要：目的探讨局部肌肉振动联合氨糖软骨素钙片治疗膝骨关节炎患者的效果,并研究其对患者关节功能和血清相关因子的影响。方法选取2017年2月至2018年8月经海口市第三人民医院骨科诊疗的84例膝骨关节炎患者,依据随机数字表法分为对照组和观察组,各42例。对照组给予氨糖软骨素钙片治疗,观察组给予局部肌肉振动联合氨糖软骨素钙片治疗,2组均治疗1个月。比较2组患者治疗前后膝关节功能(Lysholm功能评分)、疼痛视觉模拟量表(VAS)评分、血清相关因子[血清Ⅰ型胶原C端异物肽(CTX-Ⅰ)、胰岛素样生长因子(IGF)、睾酮]水平、股四头肌肌力、关节活动度及生活质量情况。结果治疗后,2组Lysholm功能评分、血清IGF和睾酮水平、股四头肌肌力、关节活动度及生活质量评分均高于治疗前,VAS评分、CTX-Ⅰ水平均低于治疗前,且观察组Lysholm功能评分、血清IGF和睾酮水平、股四头肌肌力、关节活动度及生活质量评分高于对照组[(82.9±2.9)分比(76.5±2.6)分、(172±23)mg/L比(135±23)mg/L、(0.98±0.14)ng/L比(0.71±0.17)ng/L、(4.47±0.16)级比(4.01±0.14)级、(114±5)°比(104±5)°、(80±5)分比(58±4)分],VAS评分、CTX-Ⅰ水平低于对照组[(3.2±0.5)分比(4.4±1.1)分、(0.22±0.13)mg/L比(0.36±0.15)mg/L],差异均有统计学意义(均P<0.05)。结论局部肌肉振动联合氨糖软骨素钙片治疗膝骨关节炎患者,能够较好地恢复膝关节功能情况,减轻痛苦,改善血清相关因子水平和患者预后。Objective To investigate the clinical efficacy of calcium aminochondroitin tablets combined with local muscle vibration in the treatment of knee osteoarthritis.Methods From February 2017 to August 2018,84 patients with knee osteoarthritis who were treated at department of orthopaedics Haikou Third People's Hospital were divided into control group and observation group according to random number table method,with 42 cases in each group.The control group was treated with aminochondroitin calcium tablets and the observation group was treated with local muscle vibration and aminochondroitin calcium tablets.Both groups were treated for one month.The knee joint function(Lysholm function score),pain visual analogue scale(VAS),serum related factors[C-terminal telopeptide of typeⅠcollagen(CTX-Ⅰ),insulin-like growth factor(IGF),testosterone],quadriceps muscle strength,joint mobility and quality of life were compared between the two groups before and after treatment.Results After treatment,Lysholm functional score,serum IGF,testosterone level,quadriceps muscle strength,joint activity and quality of life in the two groups were higher than those before treatment.VAS score and CTX-Ⅰlevel were lower than those before treatment.The Lysholm functional score,IGF,testosterone level,quadriceps muscle strength,joint activity and quality of life in observation group were higher than those before treatment.The Lysholm functional score,IGF,testosterone level,quadriceps muscle strength,joint activity and quality of life score in observation group were higher than those in control group[(82.9±2.9)vs(76.5±2.6),(172±23)mg/L vs(135±23)mg/L,(0.98±0.14)ng/L vs(0.71±0.17)ng/L,(4.47±0.16)vs(4.01±0.14),(114±5)°vs(104±5)°,(80±5)vs(58±4)];the VAS score and CTX-Ⅰlevel of observation group were lower than those of control group[(3.2±0.5)vs(4.4±1.1),(0.22±0.13)mg/L vs(0.36±0.15)mg/L](all P<0.05).Conclusion Local muscle vibration combined with calcium aminochondroitin tablets in the treatment of knee osteoarthritis can restore
