超前镇痛方案联合综合护理对腰椎间盘突出症减压内固定术后功能恢复及疼痛程度的影响

摘　　要：目的:探讨超前镇痛方案联合综合护理对腰椎间盘突出症减压内固定术后功能恢复及疼痛程度的影响。方法:选取2015年6月至2017年6月于该院就诊的行腰椎间盘突出症后路减压内固定术后患者112例作为研究对象,按照随机数字表法分为观察组和对照组各56例。对照组行超前镇痛联合常规护理,观察组在对照组基础上给予综合护理方案。干预2 w,对比两组患者的ODI、JOA、VAS评分及焦虑抑郁状况。结果:干预后,两组患者的ODI评分均明显降低,且观察组ODI评分明显低于对照组,差异均有统计学意义(均P<0.001);干预后,两组患者的JOA评分均明显升高,且观察组JOA评分明显高于对照组,差异均有统计学意义(均P<0.001);干预后,两组患者的VAS评分均明显降低,且观察组VAS评分明显低于对照组,差异均有统计学意义(均P<0.001);干预后,两组患者的SAS及SDS评分均明显降低,且观察组SAS及SDS评分明显低于对照组,差异均有统计学意义(均P<0.001)。结论:超前镇痛方案联合综合护理应用于腰椎间盘突出症减压内固定术后患者,显著降低了患者的疼痛程度和负面情绪,康复效果明显。Objective To explore the effect of advanced analgesia combined with comprehensive nursing on the recovery of function and pain degree after decompression and internal fixation for lumbar disc herniation.Methods A total of 112 patients with lumbar disc herniation after posterior decompression and internal fixation were selected as research objects from June 2015 to June 2017 in this hospital.According to the random number table method,they were divided into the observation group and the control group,with 56 cases in each.The control group was given advanced analgesia combined with routine nursing,and the observation group was given comprehensive nursing program on the basis of the control group.After 2 weeks intervention,Oswestry Disability Index(ODI),Japanese Orthopedics Association(JOA),Visual Analogue Scale(VAS)score,anxiety and depression were compared between the two groups.Results After intervention,ODI score in both groups was significantly reduced,and ODI score in the observation group was significantly lower than that in the control group,with statistically significant differences(P<0.001).After intervention,JOA score in both groups was significantly increased,and JOA score in the observation group was significantly higher than that in the control group,with statistically significant differences(P<0.001).After intervention,VAS score in both groups was significantly reduced,and VAS score in the observation group was significantly lower than that in the control group,with statistically significant differences(P<0.001).After intervention,SAS and SDS scores in both groups were significantly reduced,and SAS and SDS scores in the observation group were significantly lower than those in the control group,with statistically significant differences(P<0.001).Conclusions Advanced analgesia program combined with comprehensive nursing was applied to the patients with lumbar disc herniation after decompression and internal fixation,which significantly reduced the pain degree and negative emotions of the patients with
