摘 要:目的探讨母体血清及新生儿外周血中白细胞分化抗原14(sCD14)、降钙素原(PCT)、可溶性髓样细胞表达触发因子受体1(sTREM-1)及白细胞介素-10(IL-10)与胎膜早破(PROM)及新生儿围产期感染的关系。方法选取我院产科2015年1月至2018年10月建卡并发生PROM并成功分娩的产妇120例、足月妊娠产妇120例作为对照组;检测两组分娩前24h母体及新生儿出生24h血清中sCD14、PCT、sTREM-1及IL-10水平;并根据新生儿围产期是否发生感染性疾病分为感染组和非感染组进行分析。结果 PROM组产妇产前24h的血清sCD14、PCT、sTREM-1及IL-10水平显著的高于对照组(P<0.05);PROM组新生儿分娩后24h的血清sCD14、PCT、sTREM-1及IL-10水平显著的高于对照组(P<0.05);PROM组产妇中发生围产期新生儿感染母体产前24h的血清sCD14、PCT、sTREM-1及IL-10水平显著的高于非感染组(P<0.05);PROM组产妇中发生围产期新生儿感染的新生儿分娩后24h的血清sCD14、PCT、sTREM-1及IL-10水平显著的高于非感染组(P<0.05)。结论母血、新生儿外周血中sCD14、PCT、sTREM-1及IL-10水平变化对于反应胎膜早破及发生新生儿围产期感染性疾病具有一定的价值。Objective To investigate the relationship between leukocyte differentiation antigen 14(sCD14),procalcitonin(PCT),soluble myeloid cell expression trigger factor receptor 1(sTREM-1)and interleukin-10(IL-10)in maternal serum and neonatal peripheral blood and premature rupture of membranes(PROM)and perinatal infection.Methods A total of 120 cases(January 2015 to October 2018)with PROM and 120 full-term pregnant women were selected as the control group.The levels of sCD14,PCT,sTREM-1 and IL-10 in maternal and neonatal serum 24 hours before delivery were detected.The patients were divided into infection group and non-infection group according to the occurrence of infectious diseases during perinatal period.Results The serum levels of sCD14,PCT,sTREM-1 and IL-10 at 24 hours before delivery in PROM group were significantly higher than those in control group(P<0.05);the serum levels of sCD14,PCT,sTREM-1 and IL-10 at 24 hours after delivery in PROM group were significantly higher than those in control group(P<0.05);the serum levels of sCD14,PCT,sTREM-1 and IL-10 at 24 hours before delivery in PROM group were significantly higher than those in non-infected group.The serum levels of sCD14,PCT,sTREM-1 and IL-10 in PROM group were significantly higher than those in non-infection group 24 hours after delivery(P<0.05).Conclusion The changes of sCD14,PCT,sTREM-1 and IL-10 levels in maternal and neonatal peripheral blood have certain value in response to premature rupture of membranes and the occurrence of perinatal infectious diseases.
