凉血消银汤口服联合消银熏洗汤外用治疗寻常型银屑病血热证的效果及血管内皮因子的影响观察

摘　　要：目的:探究凉血消银汤口服与消银熏洗汤外用联合治疗寻常型银屑病血热证的临床效果及其应用价值.方法:随机选取北京市丰台中西医结合医院2016年10月-2019年9月中医皮肤科收治的确诊为寻常型银屑病血热证患者300例,随机数字表法分为两组,其中对照组使用凉血消银汤口服治疗,而研究组患者在对照组的基础上联合消银熏洗汤外用进行治疗,比较两组患者的临床效果、治疗前后银屑病面积与严重性指数(Psoriasis Area and Severity Index,PASI)评分、视觉模拟评分法(Visual Analogue Scale/Score,VAS)评分及血管内皮生长因子(Vascular Endothelial Growth Factor,VEGF)水平等指标.结果:研究组的临床疗效明显优于对照组(P<0.05);研究组治疗后PASI、VAS评分明显低于对照组(P<0.05);研究组治疗2周、4周VEGF水平明显低于对照组(P<0.05);研究组的复发率显著低于对照组(P<0.05).结论:凉血消银汤口服与消银熏洗汤外用联合治疗寻常型银屑病血热证,能显著降低VEGF水平,减少复发率,值得临床上推广使用.Objective:To investigate the clinical effect and application value of Liangxue Xiaoyin Decoction(凉血消银汤)combined with Xiaoyin Xunxi Decoction(消银熏洗汤)fumigation in the treatment of psoriasis vulgaris with blood heat syndrome and its effect on vascular endothelial factor.Methods:From October 2016 to September 2019,300 patients with blood-heat syndrome of psoriasis vulgare admitted to the dermatology department of traditional Chinese medicine in Beijing Fengtai Hospital of Integrated Traditional Chinese and Western medicine were randomly selected and divided into two groups by random number table method.The control group was treated orally with Liangxue Xiaoyin Decoction,while the study group was treated externally with Xiaoyin Xunxi Decoction fumigation on the basis of treatment of the control group.The clinical effect,PASIscore,VAS score and VEGF level of the two groups were compared.Results:The clinical efficacy of the study group was significantly better than that of the control group(P<0.05).PASI and VAS scores in the study group were significantly lower than those in the control group(P<0.05).VEGF levels in the study group were significantly lower than those in the control group after 2 and 4 weeks(P<0.05).The recurrence rate of the study group was significantly lower than that of the control group(P<0.05).Conclusion:Liangxue Xiaoyin Decoction combined with Xiaoyin Xunxi Decoction fumigation can significantly reduce the level of VEGF and reduce the recurrence rate of psoriasis vulgaris.
