中药直肠点滴联合穴位埋线预防及治疗伊立替康引起迟发型腹泻的效果观察

摘　　要：目的:运用中药直肠点滴联合穴位埋线预防及治疗伊立替康引起迟发型腹泻的效果观察,为临床护理工作提供参考.方法:选取晚期结、直肠癌患者应用伊立替康单药或联合其他药物化疗的患者60例,按随机数字表法分为试验组及对照组,试验组予在化疗前一天用葛根芩连汤,煎水50 mL,采取中药的直肠点滴的方法,每天1次,连用7天;大肠俞、脾俞、上巨虚、天枢穴位埋线1次;对照组患者化疗后出现腹泻的采取洛哌丁胺(易蒙停)首剂4 mg,其后每2小时2 mg,直至腹泻停止12 h以上,阿托品、抗生素预防补液等相应的对症措施.结果:试验组出现迟发型腹泻6例,均采取以上外治法,治愈6例;对照组出现迟发型腹泻的15例,治愈3例,好转5例,无效7例.两组患者迟发型腹泻的发生率(χ^2=5.934,P=0.015)及治疗效果(χ^2=4.200,P=0.040)差异均有统计学意义.结论:采用中药直肠点滴及穴位埋线在预防及治疗伊立替康导致迟发型腹泻的患者比常规西医治疗效果更佳.Objective:To observe the effect of traditional Chinese medicine rectal drip and acupoint catgut-embedding in the prevention and treatment of irinotecan-induced delayed diarrhea and to provide reference for clinical nursing.Methods:60 patients with advanced node and colorectal cancer who were treated with irinotecan alone or in combination with other drugs and chemotherapy were divided into the experimental group and the control group according to random number table.The experimental group was treated with Gegen Qinlian Decoction(葛根芩连汤)50ml and traditional Chinese medicine rectal drip method,once daily,treatment was of 7 days;in addition to acupoints catgut embedding at Dachangsu(大肠俞),Pishu(脾俞),Shangjuxu(上巨虚)and Tianshu(天枢穴);Patients in the control group who had diarrhea after chemotherapy took the first dose of imodium 4mg,followed by 2mg every 2 hours until the diarrhea stopped for 12h or more,atropine,antibiotics,preventive rehydration and other appropriate symptomatic measures.Results:In the experimental group,there were 6 cases of delayed diarrhea.All of them took the above external treatment and 6 cases were cured.In the control group,15 cases showed delayed diarrhea,3 cases were cured,5 cases improved,and 7 cases were ineffective.The incidence of delayed diarrhea(χ^2=5.934,P=0.015)and treatment effect(χ^2=4.200,P=0.040)in both groups were significantly different.Conclusion:Traditional Chinese medicine rectal drip and acupoint catgut-embedding is better than conventional western medicine in the treatment of irinotecan induced delayed diarrhea.
