静脉药物配制中心常见不合理用药医嘱分析

摘　　要：目的:通过分析重庆市开州区人民医院静脉药物配制中心对住院患者不合理用药医嘱的审核干预情况,提高合理用药水平.方法:采用回顾性检查法收集审方药师登记的2014年7月-2018年6月不合理用药医嘱干预数据,分析不合理用药现状.结果:2014年7月-2018年6月干预的不合理医嘱共9815张,占总医嘱量的0.17%(9815/5773238),其中有8208张得到了修改,占不合理医嘱总量的83.63%(8208/9815);不合理用药医嘱类型包括其他用药不适宜36.96%(3628/9815)、溶媒选择不适宜20.61%(2023/9815)、浓度不适宜15.08%(1481/9815)、剂量不适宜13.63%(1338/9815)、联合用药不适宜7.38%(725/9815)、配伍禁忌2.61%(256/9815)等.其他用药不适宜包括溶媒或药品剂量明显错误、无溶媒输入、相似药品品种错误、已停医嘱未作废等.结论:重庆市开州区人民医院静脉配制药物不合理用药总体情况发生率较低,审方药师对住院患者的长期静滴医嘱不合理用药现象能及时发现,并对不合理用药医嘱进行干预、登记、总结分析,可以提高医院的合理用药水平,保障患者用药安全.Objective:According to the analysis the Kaizhou District People's Hospital,To improve the level of rational drug use,the review and intervention of the intravenous drug allocation center on the unreasonable drug use of inpatients was analyzed.Methods:Retrospective examination was used to collect the intervention data of unreasonable medication orders from July 2014 to June 2018 registered by prescription-reviewing pharmacists,and analyze the current situation of irrational medication.Results:From July 2014 to June 2018,there were 9815 unreasonable medical orders for intervention,accounting for 0.17%(9815/5773238)of the total medical orders,among which 8208 were modified,accounting for 83.63%(8208/9815)of the total unreasonable medical orders.The types of unreasonable medication orders included inappropriate other medication 36.96%(3628/9815),inappropriate solvent selection 20.61%(2023/9815),inappropriate concentration 15.08%(1481/9815),inappropriate dose 13.63%(1338/9815),inappropriate combination medication 7.38%(725/9815),and incompatibility 2.61%(256/9815).Other inappropriate drug use included obvious wrong dosage of solvent or drug,no solvent input,wrong type of similar drug,and no invalidation of the doctor's order.Conclusion:The overall incidence of irrational drug use in intravenous configuration in our hospital is relatively low.The prescription-checking pharmacists can timely find out the irrational drug use in long-term static orders of inpatients,and conduct intervention,registration,summary and analysis of the unreasonable drug use orders,which can improve the level of rational drug use in the hospital and ensure the safety of drug use of patients.
