两种麻醉药物在老年下肢手术中的应用效果

摘　　要：目的研究等剂量罗派卡因和布比卡因对老年下肢手术患者麻醉效果及术后尿潴留的影响。方法随机数表法将本院2018年1月—2019年2月诊治的200例择期行下肢手术的老年患者分为研究组与对照组,各100例,研究组给予罗哌卡因麻醉,对照组给予布比卡因麻醉。观察比较两组患者麻醉效果和术后尿潴留情况。结果研究组患者运动阻滞和镇痛起效时间长于对照组,尿意产生时间、痛觉和运动恢复时间均明显短于对照组(P<0.05);研究组患者Bromage分级为2级的病例数明显多于对照组,3级病例数明显少于对照组(P<0.05);研究组患者术后尿滞留发生率明显低于对照组,自行排尿率明显高于对照组(P<0.05)。结论等剂量罗哌卡因应用于老年下肢手术患者麻醉效果比布比卡因更好,可有效降低术后尿滞留发生率.Objective To study the effect of equal dose of ropivacaine and bupivacaine on anesthesia and urinary retention in elderly patients undergoing lower limb surgery.Methods 200 elderly patients undergoing elective lower limb surgery in our hospital from January 2018 to February 2019 were divided into study group and control group with 100 cases in each group.The study group was given ropivacaine anesthesia and the control group was given bupivacaine anesthesia.The anesthesia effect and urinary retention were observed and compared between the two groups.Results The onset time of motor block and analgesia in the study group was longer than that in the control group,and the time of urinary intention,pain and motor recovery was significantly shorter than that in the control group(P<0.05);the number of Bromage grade 2 cases in the study group was significantly more than that in the control group,and the number of grade 3 cases was significantly less than that in the control group(P<0.05);The incidence of urinary retention was significantly lower than that of the control group,and the rate of self-urination was significantly higher than that of the control group(P<0.05).Conclusion Equivalent dose of ropivacaine is better than bupivacaine in anesthesia for elderly patients undergoing lower limb surgery,which can effectively reduce the incidence of urinary retention after operation.
