摘 要:目的:分析2016-2018年本院临床病原菌及多重耐药菌检出情况。方法:选取2016年1-12月的14 396份标本为A组,2017年1-12月的18 171份标本为B组,2018年1-12月的21 633份标本为C组。比较三组的病房及门诊标本分布情况、革兰阳性菌及革兰阴性菌占比、各类病原菌检出率及多重耐药菌检出情况。结果:C组的病房标本占比显著高于A组及B组,差异均有统计学意义(P<0.05)。B组的革兰阴性菌检出率显低于A组和C组,差异均有统计学意义(P<0.05)。三组的革兰阴性菌占比均显著高于本组革兰阳性菌占比。三组均以大肠埃希菌的检出率最高。C组大肠埃希菌的检出率高于A组及B组,B组检出率则高于A组,差异均有统计学意义(P<0.05)。A组肺炎链球菌的检出率高于B组及C组,B组检出率则高于C组,差异均有统计学意义(P<0.05)。多重耐药菌中,C组大肠埃希菌检出率高于A组及B组,C组金黄色葡萄球菌检出率则低于A组及B组,差异均有统计学意义(P<0.05)。结论:2016-2018年本院临床检出病原菌以大肠埃希菌最高,且多重耐药菌也以大肠埃希菌最高,且大肠埃希菌呈现升高的趋势,应给予充分的重视与干预。Objective:To analyze the detection situation of clinical pathogenic bacteria and multidrug resistant bacteria in the hospital from 2016 to 2018.Method:A total of 14396 specimens from January to December 2016 were selected as group A,a total of 18171 specimens from January to December 2017 were selected as group B,and A total of 21633 specimens from January to December 2018 were selected as group C.The specimen distribution,proportion of Gram-positive and Gram-negative bacteria,detection rate of various pathogenic bacteria and detection status of multi-drug resistant bacteria in the wards and outpatient clinics of the three groups were compared.Result:The proportion of ward specimens in group C was significantly higher than those in group A and group B,the differences were statistically significant(P<0.05).The detection rate of Gram-negative bacteria in group B was significantly lower than that in group A and group C,with statistically significant differences(P<0.05).The proportion of Gram-negative bacteria in the three groups were significantly higher than those in this group.The detection rate of Escherichia coli were the highest in all three groups.The detection rate of Escherichia coli in group C was higher than that in group A and group B,and the detection rate in group B was higher than that in group A,with statistically significant differences(P<0.05).The detection rate of Streptococcus pneumoniae in group A was higher than that in group B and group C,and the detection rate in group B was higher than that in group C,with statistically significant differences(P<0.05).Among the multi-drug-resistant bacteria,the detection rate of Escherichia coli in group C was higher than those in group A and group B,while the detection rate of Staphylococcus aureus in group C was lower than those in group A and group B,with statistically significant differences(P<0.05).Conclusion:From 2016 to 2018,Escherichia coli is the highest pathogenic bacteria detected in the hospital,and Escherichia coli is also the highest multi-drug
