猪肺磷脂注射液联合高频振荡通气对NRDS患儿血清CK-MB、ET-1、Cys-C的影响

摘　　要：目的:探讨猪肺磷脂注射液联合高频振荡通气(HFOV)对新生儿呼吸窘迫综合征(NRDS)患儿血清肌酸肌酶同工酶(CK-MB)、内皮素(ET-1)、血清胱抑素(Cys-C)的影响.方法:选取2018年1月-2019年1月本院收治的NRDS患儿80例,参考随机数字表法分为对照组和观察组,各40例.两组均采用基础治疗,对照组在此基础上采用HFOV治疗,观察组在对照组治疗基础上采用猪肺磷脂注射液治疗,比较两组治疗前和治疗3 d后CK-MB、ET-1、Cys-C水平、血气指标以及不良反应发生情况.结果:治疗3 d后,两组血清CK-MB、ET-1、Cys-C水平均低于治疗前,且观察组上述指标均低于对照组,差异均有统计学意义(P<0.05);治疗3 d后,两组动脉血氧分压(PaO2)、氧合指数(PaO2/FiO2)水平均高于治疗前,动脉血二氧化碳分压(PaCO2)水平低于治疗前,且观察组变化幅度均高于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:将猪肺磷脂注射液联合HFOV用于NRDS患儿中的效果较好,能有效降低血清CK-MB、ET-1、Cys-C水平,改善氧合指数,且不会增加不良反应发生率.Objective:To investigate the effects of Poractant Alfa Injection combined with high-frequency oscillatory ventilation(HFOV)on serum kinase creatase isoenzyme(CK-MB),endothelin(ET-1)and serum cystatin(Cys-C)in children with neonatal respiratory distress syndrome(NRDS).Method:Eighty children with NRDS treated in our hospital from January 2018 to January 2019 were selected and divided into the control group and the observation group according to the random number table method,40 cases in each group.Two groups were given basic treatment,the control group was treated with HFOV,and the observation group was treated with Poractant Alfa Injection based on the treatment of the control group.The levels of CK-MB,ET-1,Cys-C,blood gas indicators before and 3 d after treatment and the incidence of adverse reactions of two groups were compared.Result:3 d after treatment,the levels of serum CK-MB,ET-1,and Cys-C in two groups were lower than those of before treatment,and the above indicators in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).3 d after treatment,the arterial oxygen partial pressure(PaO2)and oxygenation index(PaO2/FiO2)levels of two groups were higher than those of before treatment,and the arterial blood carbon dioxide partial pressure(PaCO2)levels were lower than those of before treatment,and the observation group had a larger change range,the differences were statistically significant(P<0.05).The incidence of adverse reactions in two groups were compared,the difference was not statistically significant(P>0.05).Conclusion:The combined use of Poractan Alfa Injection and HFOV in children with NRDS has a better effect,it can effectively reduce the levels of serum CK-MB,ET-1 and Cys-C,improve oxygenation indexes,and will not increase the incidence of adverse reactions.
