摘  要:目的设计一种新的MRI扫描方法,为分歧韧带损伤的临床诊断提供影像学参考。方法使用Amira 5.2.2软件对5具尸体的10例分歧韧带中国数字化人体(CVH)数据进行分割和三维重建,研究分歧韧带大体形态,并对分歧韧带长度、宽度、厚度、走行角度等参数进行测量,参考测量的韧带走行角度,设计出可以完整呈现分歧韧带的斜矢状位和斜轴位扫描方法。随后选择陆军军医大学第一附属医院7例患者的10例分歧韧带行斜矢状位和斜轴位MRI扫描,并对MRI图像进行分析和比较,反证新的MRI扫描方法的有效性。结果新的MRI扫描方法可以在斜矢状位、斜轴位上良好地呈现出分歧韧带跟舟支;斜矢状位上的跟骰支呈现良好,但在斜轴位上表现不佳。断面三维重建和MRI图像上测量所得的分歧韧带跟骰支长度、宽度、厚度比较差异无统计学意义(P>0.05),而跟舟支只在远端宽度上存在统计学差异(P<0.05)。断面三维重建和MRI图像上的分歧韧带走行角度比较,差异无统计学意义(P>0.05)。从形态上看,跟舟支有扇状和索状两种形态,跟骰支较跟舟支更短、更薄、也更规则。结论基于数字化人体断面解剖三维重建模型设计的MRI扫描方法可以良好地呈现分歧韧带。Objective To design a new MRI scanning method for the clinical diagnosis of bifurcate ligament injury.Methods Amira 5.2.2 software was used to segment and reconstruct the Chinese Visible Human(CVH)data of 10 bifurcate ligaments(BLs)from 5 cadavers.The morphology of bifurcate ligaments was observed,and the length,width,thickness,angle and other parameters of bifurcate ligaments were measured.According to the measured angle of ligaments,the oblique sagittal and axial MRI scanning methods which could present bifurcate ligament completely were designed.Then 10 BLs of 7 patients from the first affiliated hospital of the Army Medical University were scanned with oblique sagittal and axial position.The effectiveness of the new scanning method was verified by analyzing and comparing the MRI images with CVH reconstruction models.Results The new MRI scanning method can present the calcaneonavicular ligament(CNL)very well in oblique sagittal and axial position,and it can present the calcaneocuboid ligament(CCL)well in oblique sagittal position,but it was unsatisfied in oblique axial position.There was no statistically significant difference in the length,width and thickness of the CCL between the CVH reconstruction models and MRI measurement(P>0.05),while the measurement of the CNL only has a statistically significant difference in the distal width(P<0.05).There was no statistically significant difference in the angle of the bifurcate ligament between the CVH reconstruction models and MRI measurement(P>0.05).In terms of morphology,the CNL has two morphologies,namely the fan-shape and the cord-shape,and the CCL is shorter,thinner,and more regular than the CNL.Conclusion The MRI scanning method based on three-dimensional visualization of undeformed high-resolution sectional anatomical images from CVH could present the bifurcate ligament satisfactorily.
