摘 要:目的观察氧化亚氮吸入镇痛在肛肠科手术中的临床应用效果。方法选取2018年9月至2019年9月接受手术治疗的肛肠疾病患者80例,随机分为对照组和试验组,每组40例。对照组给予局部注射1%利多卡因,试验组给予氧化亚氮吸入联合局部麻醉。比较2组生命体征、镇痛效果、镇静效果、舒适度、术后恢复情况、不良反应及满意度。结果2组不同时间MAP、HR、BR比较差异无统计学意义(P>0.05)。试验组术后4 h、8 h、12 h、24 h及首次排便时VAS评分、不良反应发生率显著低于对照组,RASS评分、BCS评分、满意度显著高于对照组,下床活动时间、自主排尿恢复时间、住院时间显著短于对照组,差异均有统计学意义(P<0.05)。结论肛肠科手术中应用氧化亚氮吸入镇痛效果良好,能有效发挥镇痛作用,且不良反应发生率低。Objective To investigate the postoperative analgesia effects and adverse reactions of nitrous oxide inhalation analgesia in anorectal surgery.Methods A total of 80 patients with anorectal diseases were randomly divided into control group(n=40)and experimental group(n=40).The patients in control group were given intravenous injection of 1%lidocaine,however,the patients in experimental group were given nitrous oxide inhalation combined with local anesthesia.the vital signs,analgesia effects,sedation effects,comfort degree,postoperative recovery status,adverse reactions and satisfactory degree were observed and compared between the two grousp.Results There were no significant differences in MAP,HR and BR between two groups at different time points after operation(P>0.05).The VAS scores at 4h,8h,12h,24h after operation and VAS scores at the first defecation and the incidence of adverse reactions in experimental group were significantly lower than those in control group,however,the RASS scores,BCS scores at 4h,8h,12h,24h after operation and satisfactory degree in experimental group were significantly higher than those in control group,moreover,the geting-off bed time,the recovery time of automatic micturition and the hospital stay in experimental group were significantly shorter than those in control group(P<0.05).Conclusion The nitrous oxide inhalation has better analgesia effects in anorectal surgery,which can effectively alleviate postoperative pain,with lower incidence rate of adverse reactions.
