益气活血方对心梗后左室重构大鼠心肌基质金属蛋白酶-2、肿瘤坏死因子-α的影响

摘　　要：目的:通过观察该方对心梗后左室重构(Left Ventricular Remodeling,LVRM)大鼠心梗边缘区心肌基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶组织抑制因子-2(TIMP-2)及血清肿瘤坏死因子-α(TNF-α)的影响,探讨益气活血方防治心梗后左室重构的作用机理。方法:结扎大鼠冠状动脉前降支建立心肌梗死模型,术后24 h存活大鼠分为益气活血方大剂量组(简称大剂量组)、益气活血方小剂量组(简称小剂量组)、卡托普利组(简称西药组)、模型组,另设假手术组。分别以相应药物灌胃6周。6周末检测各组大鼠左心室质量指数(LVMI),梗死边缘区心肌MMP-2、TIMP-2蛋白的表达及血清TNF-α的含量。结果:与假手术组比较,模型组6周末大鼠MMP-2、TIMP-2蛋白的表达明显增加,左心室质量指数增加;药物干预组,尤以大剂量组为优,可显著降低心肌梗死后心肌MMP-2蛋白表达,增加心肌TIMP-2蛋白表达,降低MMP-2/TIMP-2比值,降低左心室质量指数,降低血清TNF-α的含量。结论:益气活血方可抑制心肌MMP-2蛋白表达,增加心肌TIMP-2蛋白表达,降低MMP-2/TIMP-2,这可能是益气活血方抑制左室重构,防治心梗后心衰的作用机制之一,且存在量效关系。其抑制心肌MMP-2、增加心肌TIMP-2蛋白表达的作用可能跟降低血清TNF-α含量有关。Objective:Efficacy of the Yiqi Huoxue prescription(益气活血方)on mechanism of left ventricular remodeling after myocardial infarction was explored,according to efficacy of the prescription on MMP-2,TIMP-2 and TNF-α.Methods:LAD of coronary artery was ligated to establish models of AMI.And then they were divided into the large and small doses of the Yiqi Huoxue decoction group,the western medicine group,the model group and the sham operation group,and treated for 6 weeks with different therapy.Results:Compare with the sham operation group,MMP-2,TIMP-2 and LVMI in the model group were increased after 6 weeks.In high-dose the Yiqi Huoxue decoction group,the expression of MMP-2 was decreased,TIMP-2 was increased,MMP-2/TIMP-2 was decreased,and TNF-αwas decreased.Conclusion:The Yiqihuoxue decoction can inhibit the expression of MMP-2,increase the expression of TIMP-2,and decrease MMP-2/TIMP-2.This may be one of the mechanisms of the Yiqi Huoxue prescription in inhibiting left ventricular remodeling and preventing from heart failure after myocardial infarction,and there is a dose-effect relationship.Its inhibitory effect on myocardial MMP-2 and increase myocardial TIMP-2 protein expression may be related to reducing serum TNF-αcontent.
