浮针疗法联合手法治疗第三腰椎横突综合征风寒阻络型临床研究

摘　　要：目的观察浮针疗法联合手法治疗第三腰椎横突综合征风寒阻络型的临床疗效。方法将94例第三腰椎横突综合征风寒阻络型患者按照随机数字表法分为2组。对照组47例予手法治疗;治疗组47例在对照组治疗基础上加浮针疗法。2组均治疗2周。比较2组疗效;观察2组治疗前后压痛视觉模拟评分(VAS)、日本骨科协会(JOA)下腰痛评分、中医证候评分变化;比较2组治疗前后血清5-羟色胺(5-HT)、多巴胺(DA)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)水平变化。结果治疗组治愈率48.94%(23/47),对照组治愈率27.66%(13/47),治疗组疗效优于对照组(P<0.05)。2组治疗后压痛VAS均较本组治疗组降低(P<0.05),JOA下腰痛评分升高(P<0.05),且治疗组治疗后压痛VAS降低更明显(P<0.05),JOA评分升高更明显(P<0.05)。2组治疗后中医证候评分及总评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后血清5-HT、DA、IL-6、TNF-α水平均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。结论浮针疗法联合手法治疗能显著提高第三腰椎横突综合征风寒阻络型患者治愈率,降低5-HT、DA等疼痛介质水平,抑制炎性反应,缓解局部压痛,促进腰椎功能恢复。Objective To observe the effect of floating needle therapy combined with manual reduction in the treatment of third lumbar transverse process syndrome with wind-cold blocking collaterals type.Methods 94 cases of the third lumbar transverse process syndrome with wind-cold blocking collaterals type were divided into two groups according to the random number table method.47 cases in control group were treated by manual reduction.47 cases in treatment group were treated by floating needle therapy on the basis of the control group.Both groups were treated for 2 weeks.The effect of the two groups was compared.The changes of Visual analogue scoring of tenderness(VAS)score,Japanese 0rthopaedic association(JOA)score and TCM syndrome score were observed before and after treatment in the two groups.The changes of 5-hydroxytryptamine(5-HT),dopamine(DA),interleukin-8(IL-6),tumor necrosis factorα(TNF-α)in serum were compared before and after treatment in the two groups.Results The cure rate was 48.94%(23/47)in the treatment group and 27.66%(13/47)in the control group.The clinical curative effect of the treatment group was better than that of the control group(P<0.05).After treatment,the VAS score of tenderness in the two groups was lower than that in the group before treatment(P<0.05),the JOA score was increased(P<0.05).The VAS score of tenderness in the treatment group was decreased significantly(P<0.05),and the JOA score was increased significantly(P<0.05).After treatment,TCM syndrome score and total score of the two groups were lower than that of the group before treatment(P<0.05),and the treatment group was significantly lower(P<0.05).The serum 5-HT,DA,IL-6 and TNF-αin the two groups were lower than those in the group before treatment(P<0.05),and the decrease was more significant in the treatment group(P<0.05).Conclusion Floating needle therapy combined with manual reduction can significantly improve the cure rate of wind-cold blocking collaterals type of the third lumbar transverse process syndrome,and can reduce 5-HT,
