结直肠息肉患者内镜下黏膜切除术后迟发性出血诱发因素分析

摘　　要：目的:探讨结直肠息肉在内镜粘膜切除术后迟发性出血的影响因素。方法:收集于2016年1月—2019年12月期间在我院就诊的1068例结直肠息肉患者的临床资料,均于行结直肠息肉黏膜切除术,统计迟发性出血发生例数,并采用单因素方差分析和Logistic分析迟发出血因素。结果:1068例结直肠息肉患者经内镜下黏膜切除术治疗后有77例发生迟发性出血,其术后迟发性出血发生率为7.21%(77/1068)。单因素方差分析结果提示,性别、年龄、合并高血压、饮酒史、息肉形态及息肉大小差异有统计学意义(P<0.05)。Logistic分析结果提示年龄、息肉形态、息肉大小是术后迟发性出血的独立危险因素(P<0.05)。结论:患者的年龄、息肉形态及息肉直径大小是结直肠息肉在内镜粘膜切除术后引起迟发性出血的独立影响因素,临床以上应予以重点关注。Objective To study the inducting factors of delayed bleeding after endoscopic mucosal resection for colorectal polyp.Method From January 2016 to December 2019,1068 patients were selected in our hospital,which all underwent endoscopic resection of colorectal polyp and mucosa.The number of cases of delayed bleeding was counted,and the factors of postoperative delayed bleeding were analyzed by single factor and Logistic regression.Results Endoscopic resection of colorectal polyps was performed in 1068 patients,and delayed bleeding occurred in 77 patients,the incidence rate was 7.21%.The one-way anova indicated that the differences in gender,age,hypertension,history of alcohol consumption,polyp morphology and polyp size were statistically significant(P<0.05).The Logistic regression analysis suggested that age,polyp morphology and size were independent risk factors for postoperative delayed hemorrhage(P<0.05).Conclusion Age,polyp morphology and polyp diameter of patients are independent influencing factors of delayed hemorrhage caused by colorectal polyps,which should be paid more attention to clinically.
