托拉塞米联合多巴胺对老年顽固性心力衰竭的疗效及对脑钠肽的影响

摘　　要：目的探讨托拉塞米联合多巴胺对老年顽固性心力衰竭疗效及脑钠肽的影响。方法纳入我院80例2017年2月至2018年1月老年顽固性心力衰竭患者。随机数字表分组,常规组采取常规药物治疗,托拉塞米联合多巴胺辅助组采用常规药物基础上加托拉塞米联合多巴胺治疗。比较常规组、托拉塞米联合多巴胺辅助组疗效;老年顽固性心力衰竭症状得到控制的时间、平均住院的时间;治疗前后患者左心射血分数、收缩和舒张末期内径、平均血浆脑钠肽监测水平;肌肉痉挛、电解质紊乱、眩晕发生率。结果托拉塞米联合多巴胺辅助治疗组疗效、老年顽固性心力衰竭症状得到控制的时间、平均住院的时间、左心射血分数、收缩和舒张末期内径平均血浆脑钠肽监测水平相比较常规治疗组更好,P<0.05。托拉塞米联合多巴胺辅助治疗组患者的肌肉痉挛、电解质紊乱、眩晕发生率低于常规治疗组,P<0.05。结论常规药物基础上加托拉塞米联合多巴胺辅助治疗老年顽固性心力衰竭的效果理想,可改善患者的心功能,降低脑钠肽,有效减少患者的肌肉痉挛、电解质紊乱、眩晕发生率。Objective To investigate the effect of Lasaimi combined with dopamine on aged refractory heart failure and brain natriuretic peptide.Methods 80 patients with intractable heart failure were included in our hospital from February 2017 to January 2018.Random digital tables are grouped together.Conventional drug therapy is used in the regular group.Lasaimi combined with dopamine assisted group uses conventional drugs based on Jiatuolasaimi combined dopamine therapy.Comparing the effect of the conventional group and Lasaimi combined with the dopamine assistant group;The time of control and the average time of hospitalization of the senile refractory heart failure symptoms;Before and after the treatment,the left cardiac ejection fraction,contraction and diastolic end-stage inner diameter,average plasma natriuretic peptide monitoring level;Occurrence of muscle spasm,electrolyte disorder,vertigo.Results The therapeutic effect of Lasaimi combined with dopaminergic adjunctive therapy group,the time of control of senile refractory heart failure symptoms,the average length of hospitalization,left cardiac ejection score,contractile and diastolic end-stage mean blood plasma natriuretic peptide monitoring level were better than those of the conventional treatment group,P<0.05.The prevalence of muscle spasms,electrolyte disorders,and vertigo in the patients with Lasaimi combined with dopamine assisted treatment group was lower than that in the conventional treatment group,P<0.05.Conclusion Jiatuolasaimi combined with dopamine can improve the heart function,reduce the natriuretic peptide and reduce the incidence of muscle spasm,electrolyte disorder and vertigo.
