摘 要:目的:探究消渴汤对糖尿病肾病大鼠的肾小球足细胞Nephrin和Podocin表达的影响。方法:选取4周龄健康雄性Wistar大鼠45只,采用随机数字表法分为:正常组、对照组和实验组,每组各15只;对照组和实验组大鼠采用一次性腹腔注射链脲霉素(STZ)55mg/kg方法制作糖尿病肾病(DN)模型;各组大鼠分别于完成造模后第0、2、4周采用自动生化分析仪检测大鼠血肌酐、尿素氮、血糖和24 h尿蛋白含量水平;采用Western blotting检测各组大鼠肾皮质中Nephrin和Podocin蛋白表达情况;使用HE染色观察大鼠肾小球结构。结果:实验前,实验组及对照组血糖、24 h尿蛋白定量、血肌酐、尿素氮均明显高于正常组(P<0.05);实验2周后,实验组及对照组血糖、24 h尿蛋白定量、血肌酐、尿素氮均明显高于正常组(P<0.05);实验组及对照组相比各项指标均无差异(Ρ>0.05);实验4周后,实验组及对照组血糖、24 h尿蛋白定量、血肌酐、尿素氮均明显高于正常组(P<0.05);实验组与对照组相比各项指标均有下降(P<0.05);相比正常组实验组及对照组大鼠肾组织nephrin和podocin蛋白表达显著降低(P<0.05);实验组Nephrin和Podocin蛋白水平高于对照组(P<0.05)。HE染色结果显示,使用消渴汤治疗后大鼠肾小球体积相对对照组减小,肾小管间质病变减轻,纤维化和炎细胞浸润显著改善。结论:消渴汤可以通过抑制炎症、调节糖代谢并增加大鼠足细胞数目来实现治疗糖尿病肾病。Objective: To explore effect of Xiaoke Decoction on the expression of nephrin and podocin in glomerular podocytes for rats withdiabetic nephropathy(DN).Methods: 45 healthy male Wistar rats aged 4 weeks old were divided into normal group,controlled group and experimental group by random number table method,15 cases in each group.In the controlled group and the experimental group,disposable intraperitoneal injection of 55 mg/kg streptozotocin(STZ) to make DN models.At 0,2 and 4 weeks after modeling,levels of serum creatinine,blood urea nitrogen,blood glucose and 24 h urinary protein were detected by automatic biochemical analyzer.Western blotting was applied to detect expression of nephrin and podocin in renal cortex of each group.The glomerular structure was observed by HE staining.Results: Before experiment,blood glucose,24 h urine protein quantitation,serum creatinine and blood urea nitrogen in the experimental group and the controlled group were significantly higher than those in the normal group(P<0.05).At 2 weeks after experiment,the above indexes in the experimental group and the controlled group were significantly higher than those in the normal group(Ρ<0.05).There was no difference in the above indexes between the experimental group and the controlled group(Ρ>0.05).At 4 weeks after experiment,blood glucose,24 h urine protein quantitation,serum creatinine and blood urea nitrogen in the experimental group and the controlled group were significantly higher than those in the normal group(P<0.05).Compared with the controlled group,all the above indexes in the experimental group were decreased(P<0.05).Compared with the normal group,the expression of nephrin and podocin protein in renal tissue was significantly decreased in the experimental group and the controlled group(Ρ<0.05).The protein levels of nephrin and podocin in the experimental group were higher than those in the controlled group(P<0.05).The results of HE staining showed that the glomerular volume of the rats treated with Xiaoke Decoction was small
