和解汤联合耳穴贴压治疗少阳郁热型颈性眩晕疗效观察  

Effect and Prognosis Observation of Hejie Decoction Combined with Auricular Point Sticking on Shaoyang Yu-heat Type Cervical Vertigo

摘　　要：目的:观察和解汤联合耳穴贴压治疗少阳郁热型颈性眩晕的效果及预后。方法:选取2017年10月~2018年10月于我院确诊为少阳郁热型颈性眩晕患者118例进行研究,按照就诊顺序单双号进行分组,单号为对照组,双号为观察组,两组患者各59例;对照组采用常规药物联合耳穴贴压治疗,观察组采用和解汤联合耳穴贴压治疗,两组患者均治疗4周;比较两组患者治疗有效率、中医证候评分、眩晕疼痛症状评分、颈部血液流速与相关生化指标以及半年内复发率。结果:治疗后,观察组总有效率显著高于对照组(P<0.05);两组患者中医症状评分各维度均显著降低(P<0.05),且观察组显著低于对照组(P<0.05);两组患者眩晕障碍调查表(DHI)与视觉疼痛评分量表(VAS)评分均显著降低(P<0.05),且观察组显著低于对照组(P<0.05);两组患者左颈动脉(LVA)、右颈动脉(RVA)与基底动脉(BA)平均流速均显著降低(P<0.05),且观察组显著低于对照组(P<0.05);两组患者内皮素1(ET-1)均显著降低,降钙素基因相关肽(CGRP)、血浆神经肽Y(NPY)均显著增加(P<0.05),且观察组变化幅度显著高于对照组(P<0.05);观察组治疗后复发率显著低于对照组(P<0.05)。结论:和解汤联合耳穴贴压对少阳郁热型颈性眩晕患者具有很好的治疗效果,它可显著缓解患者中医证候、眩晕疼痛症状、改善患者颈部血液流速与相关生化指标,并具有较低预后复发率。Objective: To observe the effect and prognosis of Hejie Decoction combined with auricular point sticking on Shaoyang Yu-heat type cervical vertigo.Methods: A total of 118 patients who were examinedand diagnosed as Shaoyang Yu-heat type cervical vertigo in our hospital from October2017 to October 2018 were selected for the study and were grouped according to the single and double numbers of treatment sequence.The patients with single number were in the controlled group and the patients with double number were in the observation group,with 59 cases in each group.The controlled group was given conventional drugs combined with auricular point sticking,and the observation group was given Hejie Decoction combined with auricular point sticking,and the two groups were treated for 4 weeks.The effective rate of treatment,TCM syndromes scores,vertigo pain symptoms scores,neck blood flow velocity and related biochemical indicators and recurrence rate within six months were compared between the two groups.Results: After treatment,the total effective rate in the observation group was significantly higher than that in the controlled group(P<0.05).The TCM symptoms scores in the two groups were significantly decreased(P<0.05),and the scores in the observation group were significantly lower than those in the controlled group(P<0.05).The scores of Dizziness Handicap Inventory(DHI) and Visual Analogue Scale(VAS) were significantly decreased in the two groups(P<0.05),and the scores in the observation group were significantly lower than those in the controlled group(P<0.05).The mean flow velocities of left carotid artery(LVA),right carotid artery(RVA) and basilar artery(BA) were significantly decreased in the two groups(P<0.05),and the velocitiesh in the observation group were significantly lower than those in the controlled group(P<0.05).The level of endothelin-1(ET-1) in the two groups was significantly decreased while the levels of calcitonin gene-related peptide(CGRP) and plasma neuropeptide Y(NPY) were significantly increased(P<0
