肘关节松解术联合铰链外固定架治疗肘关节粘连的疗效

摘　　要：目的探讨肘关节松解术联合铰链外固定架治疗肘关节粘连的疗效。方法将60例肘关节粘连患者随机分为对照组与观察组,每组各30例。对照组予以单纯肘关节松解术治疗,观察组予以肘关节松解术联合铰链外固定架治疗。记录两组术后生活质量评分、疼痛VAS评分、住院时间、功能恢复时间及术后不良反应;采用Broberg-Morrey肘关节功能评分和Mayo肘关节功能评分评估术后肘关节功能,采用屈伸活动范围评估肘关节活动程度。结果患者均随访6个月。住院时间、功能恢复时间对照组均长于观察组(P<0.05)。术后6个月,Broberg-Morrey肘关节功能评分优良率对照组为46.7%,低于观察组的76.7%(P<0.05);肘关节Mayo评分及屈伸活动范围对照组均低于观察组(P<0.05);生活质量评分对照组低于观察组(P<0.05)。两组随访期间均未出现不良反应。结论肘关节松解术联合铰链外固定架可显著提高肘关节粘连松解疗效,促使肘关节功能恢复并提高生活质量。Objective To explore the clinical curative effect of elbow arthrolysis combined with hinge external fixator in the treatment of elbow adhesions.Methods A total of 60 patients with elbow adhesions were divided into the control group(n=30)and the observation group(n=30).The control group were treated by elbow arthrolysis alone,while the observation group were treated with elbow arthrolysis combined with hinge external fixator.The quality of life,pain VAS,hospital stay and functional recovery time of the two groups were recorded,and adverse reactions during follow-up period were also recorded.The elbow function and the range of motion of elbow were evaluated with Broberg-Morrey score,Mayo elbow performance score and the residual extension-flexion range of motion(ROM).Results All patients were followed up for 6 months.The length of hospital stay and functional recovery time were significantly longer than the observation group(P<0.05).At 6 months postoperation,the excellent and good rate of Broberg-Morrey score in the control group(46.7%)was significantly lower than 76.7%of the observation group(P<0.05).The Mayo score and residual extension-flexion ROM of the control group were significantly lower than the observation group(P<0.05).The life quality score of the control group was significantly lower than that of the observation group.There were no adverse reactions in the two groups.Conclusions Elbow arthrolysis combined with hinge external fixator can significantly improve the curative effect of elbow arthrolysis,promote the recovery of elbow function and quality of life.
