摘  要:目的探讨股骨近端防旋髓内钉(PFNA)治疗老年股骨转子间骨折的疗效。方法采用PFNA治疗80例老年股骨转子间骨折患者。术后6个月采用Sanders评分评价髋关节功能。结果患者均获得随访,时间6~24个月。骨折均愈合良好,未出现髋内翻、断钉、防旋螺钉切出等并发症。1例术后3 d因感冒发生肺性脑病,1例术后4 d发生心肌梗死,2例术后1个月发生脑梗死,该4例经积极治疗后均明显好转。术后6个月按照创伤性髋关节Sanders评分评价疗效:优59例,良14例,可6例,差1例,优良率91.25%。结论对于老年股骨转子间骨折,PFNA固定具有创伤小、固定牢固、术后髋关节功能恢复好等优点。Objective To investigate the effect of proximal femoral anti-rotation nail(PFNA)in the treatment of intertrochanteric fractures of femur in the elderly.Methods The 80 cases of intertrochanteric fracture of femur were treated with PFNA.The hip function was evaluated by Sanders score at 6 months postoperatively.Results All patients were followed up from 6 months to 24 months.All the fractures were healed well,and no complications such as coxa vara,broken screws,and anti-rotation nail cut-out occurred.One case had pulmonary encephalopathy due to cold at 3 d after operation,one case had myocardial infarction at 4 d after operation,and two cases had cerebral infarction at 1 month after operation,the 4 cases were all improved after active treatment.At 6 months after operation,the curative effect was evaluated by Sanders score of traumatic hip joint:excellent in 59 cases,good 14 cases,fair 6 cases,poor 1 case,the excellent and good rate was 91.25%.Conclusions For intertrochanteric fractures of femur in the elderly,PFNA fixation has the advantages of less trauma,more effective,firm fixation,and good recovery of hip function.
