摘 要:目的探索骨保护素(OPG)/脱蛋白骨(DPB)复合物对兔前交叉韧带(ACL)重建后腱-骨愈合的影响。方法制备OPG/DPB复合物。取健康新西兰大白兔60只,随机分为4组(n=15):OPG组、DPB组、OPG/DPB组及对照组,均建立ACL重建模型,腱-骨界面分别植入OPG、DPB及OPG/DPB,对照组旷置处理。术后4、8、12周取材行大体、组织学观察和生物力学检测。结果组织学观察显示,术后4周时,各组均未见Sharpey样胶原纤维,由纤维结缔组织填充;术后8周时,OPG/DPB组Sharpey样胶原纤维最为明显;术后12周时,OPG/DPB组可见典型4层结构。生物力学测试显示,术后4周各组最大加载负荷无明显差异(P>0.05);术后8周,OPG/DPB组最大加载负荷最高,与其他各组差异显著(P<0.05);术后12周,各组差异无统计学意义(P>0.05)。结论OPG/DPB复合物可显著促进ACL重建术后的腱-骨早期愈合。Objective To explore the effects of osteoprotegerin(OPG)combined with deproteinized bone(DPB)on tendon-bone healing after anterior cruciate ligament(ACL)reconstruction in rabbit.Methods The complex of OPG/DPB was prepared.Sixty experimental rabbits were randomly divided into 4 groups(n=15):OPG,DPB,OPG/DPB and control group,ACL was reconstructed in all the groups,and the bone tunnels were filled with OPG,DPB,OPG/DPB correspondingly except control group.The pathological observation and histological observation were performed and the biomechanical properties were measured at 4,8 and 12 weeks after operation.Results Histological observation showed that at 4 weeks after operation,no sharpey-like collagen fibers were found in each group,which were filled by fibrous connective tissue.At 8 weeks after operation,sharpey-like collagen fibers were most obvious in OPG/DPB group.At 12 weeks after operation,the atypical 4-layer structure of bone was seen in OPG/DPB group.The biomechanics test showed that the maximum load difference among the groups was not statistically significant at 4 weeks after operation(P>0.05).At 8 weeks after operation,the maximum load of OPG/DPB group was the highest,and the difference was statistically significant(P<0.05).At 12 weeks after operation,there was no significant difference among the groups(P>0.05).Conclusion The complex of OPG/DPB can promote early tendon-bone healing after ACL reconstruction.
