假体置入联合自体脂肪移植隆乳术在临床中的应用

摘　　要：目的探讨假体置入联合自体脂肪移植一期隆乳术应用于被覆软组织较薄患者的可行性、手术方法及临床效果。方法经腋窝切口分离出胸大肌后间隙,置入硅胶假体;从双下肢大腿部后侧脂肪堆积处采集自体脂肪组织,离心处理后均匀注射于乳房的各个区域,以达到良好的软组织增厚和假体覆盖,形成自然的外观和手感。随访期间对乳房的形态、假体的轮廓感及手感进行评价。结果术后随访3~18个月,平均7个月。本组173例患者中,除1例一侧乳房局部出现较明显的脂肪结节外,余均未出现脂肪液化、坏死等并发症;术后乳房形态、大小、手感和位置的满意率达92%。结论假体置入联合脂肪移植隆乳术,可防止假体轮廓显现,术后乳房形态美观、手感自然,在被覆软组织较薄患者中应用有良好的临床效果。Objective To explore the feasibility,surgical technique and clinical result of combined use of implant and fat grafting for breast augmentation in patients with thin soft tissue.Methods A transverse incision was made in the axilla and the dissection was performed under the pectoral muscle,a silicone prosthesis was then inserted into the submuscular pocket;Fat was harvested from bilateral thighs,and after centrifugal treatment,it was injected into multiple regions in the breasts,in order to achieve soft tissue thickening and good coverage of the prosthesis thus giving a natural look and feel.During the follow-up period,shape of the breasts,visibility and palpability of the prosthesis were observed.Results All the patients were followed up for 3 to 18 months,7 months on average.Among the 173 patients,only 1 patient was reported to have a hard nodule formation in one of the breasts.There were no other complications such as fat liquefaction,necrosis.Patients satisfaction in terms of shape,size,feel and position of the prosthesis was 92%.Conclusion Prosthesis implantation combined with autologous fat transplantation can prevent implant visibility and palpability,and can improve the aesthetic outcome resulting in more natural looking and feeling breasts.The clinical results are satisfactory in patients with thin soft tissue over their breasts.
