缬沙坦联合氢氯噻嗪治疗老年高血压的疗效及其作用机制

摘　　要：目的探讨缬沙坦联合氢氯噻嗪治疗老年高血压的疗效及其作用机制。方法选取2016年3月至2018年4月92例老年高血压患者,按就诊先后分成观察组(n=46)与对照组(n=46),对照组采取缬沙坦治疗,观察组采取缬沙坦联合氢氯噻嗪治疗,观察两组治疗效果、24 h动态血压值、血浆NO、内皮素水平、治疗后左心室重量指数(LVMI)与左心室肌质量(LVM)及不良反应。结果观察组患者治疗总有效率为93.48%,明显高于对照组63.04%,差异有统计学意义(P<0.05);两组患者24 h治疗后舒张压、收缩压均低于治疗前,差异有统计学意义(P<0.05);观察组治疗后舒张压、收缩压均低于对照组,差异有统计学意义(P<0.05);观察组治疗后LVMI、LVM指数均低于对照组,差异有统计学意义(P<0.05);观察组治疗后血浆NO水平高于对照组,内皮素水平低于对照组,差异有统计学意义(P<0.05);两组不良反应对比无统计学差异(P>0.05)。结论缬沙坦联合氢氯噻嗪治疗老年高血压的疗效显著,可有效控制血压,改善内皮功能,缓解左心室肥厚程度,不良反应少,临床应用价值高。Objective To investigate the effi cacy and mechanism of valsartan combined with hydrochlorothiazide in the treatment of senile hypertension.Methods 92 elderly patients with hypertension from March 2016 to April 2018 were divided into observation group(n=46)and control group(n=46)according to the treatment order.The control group was treated with valsartan,while the observation group was treated with valsartan and hydrochlorothiazide.The therapeutic effect,24-hour ambulatory blood pressure,plasma NO,endothelin level,left ventricular mass index(LVMI)and left ventricular myocardium were observed after treatment.Quality(LVM)and adverse reactions.Results The total effective rate of the observation group was 93.48%,signifi cantly higher than that of the control group(63.04%,P<0.05);the diastolic and systolic blood pressures of the two groups after 24 hours treatment were lower than those before treatment,the difference was statistically signifi cant(P<0.05);the diastolic and systolic blood pressures of the observation group after treatment were lower than those of the control group,the difference was statistically signifi cant(P<0.05);LVM index and LVM index were lower than those of the control group,the difference was statistically signifi cant(P<0.05);plasma NO level in the observation group was higher than that of the control group after treatment,and endothelin level was lower than that of the control group,the difference was statistically signifi cant(P<0.05);there was no signifi cant difference in adverse reactions between the two groups(P>0.05).Conclusion Valsartan combined with hydrochlorothiazide is effective in the treatment of senile hypertension.It can effectively control blood pressure,improve endothelial function,alleviate left ventricular hypertrophy,with fewer adverse reactions and high clinical value.
