初始低剂量左甲状腺素治疗先天性甲状腺功能减低症的疗效观察

摘　　要：目的探讨初始低剂量左甲状腺素治疗先天性甲状腺功能减低症的疗效。方法收集我院90例2017年4月至2018年1月先天性甲状腺功能减低症患者。简单随机化方法分初始常规剂量左甲状腺素治疗组44例和初始低剂量左甲状腺素治疗组46例比较效果。结果初始低剂量左甲状腺素治疗组疗效44例(95.65%)高于初始常规剂量左甲状腺素治疗组32例(72.73%),P<0.05。治疗前两组相关的甲状腺功能血清监测指标三碘甲状腺原氨酸、甲状腺素以及促甲状腺素并无明显差异,P>0.05;治疗后初始低剂量左甲状腺素治疗组相关的甲状腺功能血清监测指标三碘甲状腺原氨酸、甲状腺素以及促甲状腺素优于初始常规剂量左甲状腺素治疗组,P<0.05。初始低剂量左甲状腺素治疗组患儿智能发育情况评分优于初始常规剂量左甲状腺素治疗组,P<0.05,初始低剂量左甲状腺素治疗组不良反应低于初始常规剂量左甲状腺素治疗组,P<0.05。结论初始低剂量左甲状腺素治疗先天性甲状腺功能减低症的疗效确切。Objective To investigate the effi cacy of initial low dose levothyroxine in the treatment of congenital hypothyroidism.Methods 90 patients with congenital hypothyroidism in our hospital from April 2017 to January 2018 were collected.A simple randomization method was used to compare the effi cacy of 44 patients in the treatment group with initial routine dose of levothyroxine and 46 patients in the treatment group with initial low dose of levothyroxine.Results The effi cacy of the initial low dose levothyroxine treatment group was 44 case(95.65%)higher than that of the initial conventional dose levothyroxine treatment group 32 case(72.73%),P<0.05.Before treatment,there were no significant differences between the two groups in the serum monitoring indexes of thyroid function,including triiodothyrogen,thyroxine and thyrotropin(P>0.05).After treatment,serum monitoring indexes of thyroid function including triiodothyrogen,thyroxine and thyrotropin in the initial low dose levothyroxine treatment group were better than those in the initial conventional dose levothyroxine treatment group(P<0.05).The score of intelligence development of children in the initial low dose levothyroxine treatment group was better than that in the initial conventional dose levothyroxine treatment group(P<0.05),and the adverse reactions in the initial low dose levothyroxine treatment group were lower than that in the initial conventional dose levothyroxine treatment group(P<0.05).Conclusion Initial low dose levothyroxine is effective in the treatment of congenital hypothyroidism.
