彩色多普勒超声诊断联合内皮素1(ET-1)检测对诊断肝硬化门静脉高压症的临床意义

摘　　要：目的研究超声诊断联合内皮素(ET-1)检测对诊断肝硬化门静脉高压症的临床意义。方法选择来我院就诊并确诊为肝硬化患者为研究对象,其中男性200例,女性80例,共280例患者,采用彩色多普勒超声技术检测肝硬化患者肝脏参数的血流动力学变化,采用放射免疫法检测血管活性物质ET-1含量的变化,研究血流动力学变化与ET-1的关系。结果与对照组相比肝硬化门静脉系统血流动力学异常,肝硬化患者门静脉直径(Dpv)和脾静脉直径(Dsv)更宽(P<0.05),脾静脉量(Qsv)显着增加(P<0.01),门静脉速度(Vpv)逐渐下降,脾静脉速度(Vsv)无显著变化。血浆ET-1含量明显增加,与肝硬化程度相关。此外,血清ET-1含量与Dpv,Dsv和Qsv之间存在正相关关系,与Vpv呈负相关。结论彩色多普勒超声技术检测门静脉和脾静脉异常血流动力学及血清ET-1水平可作为诊断高血压的手段之一。Objective To investigate The clinical signifi cance of color Doppler ultrasound combined with endothelin-1(ET-1)detection in e diagnosis of cirrhotic portal hypertension.Methods The patients who were admitted to our hospital and diagnosed as cirrhosis were included in the study.There were 200 males and 80 females.The hemodynamic changes of liver parameters were detected by color Doppler ultrasound.The changes of vascular active substance ET-1 content were detected by radioimmunoassay.The relationship between hemodynamic changes and ET-1 was studied.Results Compared with the control group,the hemodynamics of the cirrhotic portal system was abnormal.The portal vein diameter(Dpv)and splenic vein diameter(Dsv)were wider in patients with cirrhosis(P<0.05).The amount of splenic vein(Qsv)was signifi cantly increased(P<0.01)and the portal vein velocity(Vpv)gradually decreased.While the splenic vein velocity(Vsv)showed no change.Plasma ET-1 levels increased signifi cantly,which was related to the degree of cirrhosis.In addition,there was a positive correlation between serum ET-1 content and Dpv,Dsv and Qsv and a negative correlation with Vpv.Conclusion Color Doppler ultrasonography detected abnormal hemodynamics in portal vein and splenic vein and serum ET-1 levels was served as one of the means to diagnose hypertension.
