摘  要:目的探究骨髓细胞形态检验在难治性贫血和巨幼细胞性贫血鉴别中的应用效果。方法选取2017年10月至2019年8月我院收治的40例难治性贫血(RA)患者和39例巨幼细胞性贫血(MA)患者作为本次研究的主要观察对象,即RA组和MA组,所有患者入院后均进行骨髓细胞形态检验。对比两组贫血患者的检测结果。结果RA贫血组患者粒系病态39例,粒系巨幼变37例,红系病态31例,红系幼巨变34例,多核小巨核32例,淋巴样小巨核36例,MA贫血组患者粒系病态2例,粒系巨幼变36例,红系病态1例,红系幼巨变34例,多核小巨核31例,淋巴样小巨核2例。RA贫血组患者粒系病态、红系病态、淋巴样小巨核所占比(97.50%、77.50%、90.00%)均高于MA贫血组患者(5.13%、2.56%、5.13%),差异具有统计学意义(P<0.05)。MA贫血组患者的巨幼变早幼细胞大小为(21.73±2.13)μm,巨幼变原始红细胞大小为(26.07±3.38)μm,巨幼变中幼红细胞大小为(14.32±1.12)μm。RA贫血组患者的巨幼变早幼细胞大小为(17.87±2.64)μm,巨幼变原始红细胞大小为(20.45±3.46)μm,巨幼变中幼红细胞大小为(13.87±1.03)μm,MA贫血组患者的巨幼变早幼细胞及巨幼变原始红细胞的胞体明显大于RA贫血组患者,差异具有统计学意义(P<0.05)。结论RA和MA的骨髓细胞形态检验结果存在显著差异,临床对于RA的诊断可以将粒系病态、红系病态、淋巴样小巨核占比作为观察点,将巨幼变早幼细胞和巨幼变原始红细胞的胞体大小作为MA的诊断观察重点。Objective Explore the analysis of the application of bone marrow cell morphology test in the identification of refractory anemia and megaloblastic anemia.Methods Forty patients with refractory anemia(RA)and 39 patients with megaloblastic anemia(MA)treated in our hospital from October 2017 to August 2019 were selected as the main observation objects of this study,namely the RA group and MA Group,all patients were examined for bone marrow cell morphology after admission.The results of the anemia patients were compared between the two groups.Results There were 39 patients with granulocytic disease in the RA anemia group,37 patients with granulocytic megaloblastism,31 patients with the red blood cell megaloblastic disease,34 patients with multinucleated small giant nucleus,36 lymphoid micromeganuclei,and 36 patients with MA anemia group.Two cases were morbid,36 cases were granulomegaloblastic changes,1 case was erythroblastic changes,34 cases were red juvenile changes,31 were multinucleated small giant nuclei,and 2 were lymphoid small giant nuclei.The proportion of granulocytic disease,erythroid disease,and lymphoid micromeganucleus(97.50%,77.50%,90.00%)in RA anemia group were higher than those in MA anemia group(5.13%,2.56%,5.13%)(P<0.05).The size of the megaloblastic early juvenile cells in the MA anemia group was(21.73±2.13)μm,the size of the primordial red blood cells of the megaloblast was(26.07±3.38)μm,and the size of the young red blood cells in the megaloblasts was(14.32±1.12)μm.The size of megaloblastic early juvenile cells in patients with RA anemia was(17.87±2.64)μm,the size of primordial red blood cells of megaloblasts was(20.45±3.46)μm,and the size of juvenile red blood cells in megaloblasts was(13.87±1.03)μm,MA The cells of megaloblastic early progenitor cells and megaloblastic primitive red blood cells in the anemia group were significantly larger than those in the RA anemia group(P<0.05).Conclusion There are significant differences in bone marrow cell morphology test results between RA and
