益气泄浊方治疗慢性肾脏病Ⅲ~Ⅳ期气虚湿浊证患者的疗效分析

摘　　要：目的观察益气泄浊方对慢性肾脏病Ⅲ~Ⅳ期气虚湿浊证患者的临床疗效。方法选取2018年2月至7月慢性肾脏病Ⅲ~Ⅳ期气虚湿浊证患者94例,随机分为观察组48例和对照组46例。对照组采用西医基础治疗,观察组在对照组治疗的基础上予以益气泄浊方口服治疗,两组疗程均为3个月。比较两组患者主要疗效指标、尿毒症毒素水平及近期临床疗效,中医症候积分及评估益气泄浊方的血清安全性。结果观察组总有效率高于对照组(66%vs.56.3%),差异有统计学意义(P<0.05)。观察组BUN、Scr、血β2-MG、血CysC水平显著降低,与对照组比较,差异有统计学意义(P<0.05),两组患者中医症状积分均较前下降,观察组与对照组比,差异有统计学意义(P<0.05),治疗后,两组血清安全性指标(ALT、AST、Hb、Alb)差异无统计学意义(P>0.05)。结论口服益气泄浊方能有效清除慢性肾脏病Ⅲ~Ⅳ期患者尿毒症毒素,能够保护肾功能,能有效改善患者的中医症候积分,且未见明显副作用。Objective To conduct a prospective randomized clinical trial to observe the effect of Yiqi xiezhuo Decoction in patients with chronic kidney disease of stage 3 and 4,which belonged to Qi deficiency and damp turbidity pattern in TCM.Methods 94 patients with chronic kidney disease of stage 3 and 4,which belongs to Qi deficiency and damp turbidity pattern in TCM were randomly divided into Yiqi xiezhuo Decoction group(treatment group)and conventional treatment group(control group)with 3 months of treatment course from February 2018 to July 2018,48 patients in treatment group and 46 patients in control group.The control group gave conventional basic treatment,while Yiqi xiezhuo Decoction group gave conventional basic treatment added Yiqi xiezhuo Decoction.Then the primary treatment indexes and the serum level of uremic toxin and short-term clinical effect of two groups were compared.In addition,the indexes also involved the TCM syndrome scores and serum safety indicators of Yiqi xiezhuo Decoction.Results The total effective rate of treatment group was higher than control group(66%vs.56.3%)and the difference was statistically significant(P<0.05).The serum uremic toxins(BUN,Scr,β2-MG and CysC)were significantly lower in the treatment group,the difference was statistically significant compared with the control group(P<0.05).In addition,the TCM symptom scores of patients in the two groups were both decreased compared with those before the treatment,and the difference between the treatment group and the control group was statistically significant(P<0.05).However,no significant difference was observed between the treatment group and control group for the serum safety indicators(ALT,AST,Hb and Alb)(P>0.05).Conclusions The application of oral Yiqi xiezhuo Decoction can contribute to removal of uremic toxins,stabilization of renal function,and improvement of the TCM syndrome scores and clinical symptoms without any serious adverse reactions.
