单中心综合重症监护病房曲霉菌肺部感染的调查与干预

摘　　要：目的对医院单中心综合重症监护病房曲霉菌肺部感染病例进行流行病学调查,为曲霉菌医院感染可能的传播途径和寻找有效的预防与控制方法提供依据。方法采用流行病学调查,对2019年10月—12月期间出现3例曲霉菌肺部感染患者进行追踪调查,患者痰液、血液及所在环境卫生学进行目标微生物培养。结果3例患者临床症状以发热、咳嗽、咳痰等症状,其中1例有咯血症状,胸部影像学表现为结节、团块、斑片及空洞浸润影等症状。加强培训医务人员的无菌操作技术,患者周围环境的清洁与消毒。3例曲霉菌肺部感染的患者从痰液中培养结果出黄曲霉菌,与2号患者使用的CRRT按键检测出的黄曲霉一致。结论医院环境物体表面污染严重,提高手卫生依从性、严格无菌技术操作及加强水源性等医疗环境表面的清洁与消毒,是避免医院曲霉菌肺部感染发生的有效措施。Objective To investigate three cases of pulmonary infection caused by Aspergillus in a single center intensive care unit from October 1 to December 31,2019.So as to provides the basis for the possible transmission route and the effective prevention and control method of Aspergillus infection in hospital.Methods Totally 3 cases of pulmonary Aspergillosis were followed up by epidemiological investigation.The sputum,blood and environmental hygiene of the patients were examined by target microorganism culture.Results The clinical symptoms of 3 patients were fever,cough and expectoration,1 patient had hemoptysis,and chest imaging showed nodules,masses,plaques and cavity infiltration.Strengthen the training of medical staff aseptic operation technology.Strengthen the cleaning and disinfection of the surface of the medical environment.Aflatoxin was detected from sputum culture in 3 patients with pulmonary Aspergillus,which was consistent with that detected by CRRT button in patient 2.Conclusion The surface pollution of the objects in the hospital environment is serious.Improving the hand hygiene compliance,strict aseptic technique operation and strengthening the disinfection of the surface of the medical environment are the effective measures to avoid the pulmonary infection of aspergillus in the hospital.
