带状疱疹合并Ogilvie综合征2例临床分析

摘　　要：目的探讨分析河南省人民医院皮肤科收治的2例带状疱疹并发Ogilvie综合征的发病原因、临床特点、诊断、治疗及预后,并对目前已发表的带状疱疹并发Ogilvie综合征的国内外相关文献资料进行总结及回顾分析,以提高临床中对此病的认识,避免误诊误治,降低发生肠穿孔等严重并发症的风险,减少此病引起的死亡率。方法河南省人民医院2017年9月—2019年9月共收治550余例带状疱疹病人,其中2例带状疱疹并发有Ogilvie综合征即急性假性结肠梗阻,治疗上给予一般抗病毒、止痛、营养神经等治疗的同时,积极给予禁食、胃肠减压、补液、营养支持等综合性保守治疗。结果通过积极对症治疗,2例患者急性假性肠梗阻的症状缓解,随访1个月病情均未复发。结论腰腹部带状疱疹可并发Ogilvie综合征,临床工作中需予以警惕,早期识别,及时给予对症治疗,减少肠穿孔等胃肠道严重并发症的风险,避免不必要手术,降低死亡率。Objective To analyze the etiology,clinical characteristics,diagnosis,treatment and prognosis of 2 cases of herpes zoster complicated with Ogilvie syndrome in the Department of Dermatology,Henan Provincial People's Hospital.In order to improve the clinical understanding of herpes zoster complicated with Ogilvie syndrome,avoid misdiagnosis and mistreatment,and reduce the risk of serious complications such as intestinal perforation,the relevant literatures of herpes zoster complicated with herpes zoster syndrome were summarized and analyzed retrospectively.Reduce the death rate caused by the disease.Methods More than 550 patients with herpes zoster were treated in our hospital from September 2017 to September 2019.Among them,2 cases of herpes zoster complicated with Ogilvie syndrome,that is,acute pseudocolon obstruction.In the treatment of herpes zoster,we should not only treat herpes zoster with anti-virus,analgesia and nerve nutrition,but also give comprehensive conservative treatment,such as fasting,gastrointestinal decompression,fluid replacement,nutritional support and so on.Results After active symptomatic treatment,the symptoms of acute pseudointestinal obstruction were relieved in 2 patients,and there was no recurrence after 1 month follow-up.Conclusion Herpes zoster of waist and abdomen can be complicated with Ogilvie syndrome.It is necessary to be vigilant in clinical work,early identification,timely symptomatic treatment,reduce the risk of gastrointestinal complications such as intestinal perforation,avoid unnecessary surgery and reduce mortality.
