摘　　要：目的观察胃转流术(GBP)对Goto-Kakizaki(GK)/Wister大鼠脂肪组织中葡萄糖转运体4(GLUT-4)和富含半胱氨酸的酸性分泌蛋白(SPARC)表达的影响,探讨GBP改善胰岛素抵抗(IR)的可能机制。方法将健康雄性GK大鼠随机分为糖尿病手术组(DO组,行GBP)、糖尿病假手术组(DS组,行假手术)和糖尿病对照组(DC组,不做任何处理),另设健康雄性Wister大鼠作为正常对照组(NC组,不做任何处理),每组8只。分别于术前及术后第1、2、4和8周记录4组大鼠的体质量,检测空腹血糖(FPG)、空腹血清胰岛素(Fins)和糖化血红蛋白(HbA1c)水平,并计算术前及术后第8周时的胰岛素敏感指数(QUICKI);采用Western blot法检测4组大鼠术后第8周脂肪组织中GLUT-4蛋白和SPARC蛋白的表达水平。结果①体质量:DO组大鼠术后第2周和4周的体质量与术前相比均降低(P<0.05),而术后第8周时体质量逐渐上升至正常水平,与术前比较差异无统计学意义(P>0.05);术后各时点DO组大鼠的体质量低于同时间点DS、DC和NC组(P<0.05)。②FPG:DO组大鼠术后第2、4和8周的FPG水平与术前相比均较低(P<0.05);DO组大鼠自术后第2周开始FPG水平低于DS组和DC组(P<0.05),而DS组和DC组大鼠各时间点的FPG水平比较差异均无统计学意义(P>0.05)。③Fins:DO组大鼠术后第2周时的Fins水平高于术前(P<0.05),第4周和第8周逐渐下降至正常水平,同时点与NC组比较差异不大;术后第2周时,DO组大鼠的Fins水平高于DS组和DC组(P<0.05),而DS组和DC组大鼠的Fins水平比较差异无统计学意义(P>0.05)。④HbA1c:与术前比较,DO组大鼠自术后2周开始HbA1c水平有逐渐下降趋势,但术后各时点与术前比较差异均无统计学意义(P<0.05);术后第8周时,4组大鼠的HbA1c水平比较差异无统计学意义(P>0.05)。⑤QUICKI:与术前比较,术后8周时DO组大鼠的QUICKI值上升,差异有统计学意义(P<0.05);术后8周时,DO组和NC组大鼠的QUICKI值较DS组和DC组高,差异有Objective To investigate the expressions of glucose transporter-4(GLUT-4)and secreted protein acidic and rich in cysteine(SPARC)in adipose tissue of Goto-Kakizaki(GK)/Wister rats after gastric bypass operation(GBP),and to explore the possible mechanism of GBP improving insulin resistance.Methods Healthy male GK rats were randomly divided into diabetic operation group(DO group,underwent GBP),diabetic sham operation group(DS group,underwent sham-operation),and diabetic control group(DC group,received no-treatment),Wister rats were set as normal control group(NC group,received no-treatment).The weight,fasting blood glucose(FPG),fasting serum insulin(Fins),and HbA1c were measured before operation and at the 1st,2nd,4th,and 8th week after operation.Quantitative insulin sensitivity check index(QUICKI)was measured in before operation and at the 8th week after operation,and the expressions of GLUT-4 and SPARC protein in adipose tissues were measured by Western Blot method at the 8th week after operative too.Results①Weight:the weight of the DO group was lower than preoperative at the 2nd and 4th week after GBP(P<0.05),but increased to the normal level at the 8th week after GBP(P>0.05).The weight of the DO group was lower than those of the DS group,DC group,and NC group at the same time point of 2nd,4th,and 8th week(P<0.05).②FPG:the FPG level of the DO group was lower than preoperative at the 2nd,4th,and 8th week after GBP(P<0.05),and lower than those of the DS group and the DC group from 2nd to 8th week after GBP(P<0.05).The FPG level between the DS group and the DC group had no statistical significance(P>0.05).③Fins:the Fins level of the DO group was higher than preoperative at the 2nd week after GBP(P<0.05),and decreased gradually at 4th and 8th week but not significantly differed from the NC group at the same time point.At the 2nd week after GBP,the Fins level of the DO group was higher than those of the DS group and the DC group(P<0.05),but there was no statistical significance between the DS group and the DC gr
