经后路椎弓根钉棒系统治疗强直性脊柱炎合并腰椎骨折临床分析

摘　　要：目的:分析经后路椎弓根钉棒系统治疗强直性脊柱炎合并腰椎骨折临床效果.方法:选取南方医科大学珠江医院2018年1月-2019年6月收治的强直性脊柱炎合并腰椎骨折患者32例,随机分为对照组和观察组(每组16例),对照组给予传统的椎弓根螺钉内固定术治疗,观察组给予经后路椎弓根钉棒系统治疗,观察2 组患者治疗前后椎体前后缘高度、椎管截面积、Cobb's角及脊髓神经损伤情况.结果:治疗后2组患者椎管截面积、Cobb's角及椎体前后缘高度均得到了改善,且观察组改善情况优于对照组,差异具有统计学意义(P<0.05);治疗后2组患者脊髓神经损伤均得到了改善,比较差异间无统计学意义(P>0.05).结论:经后路椎弓根钉棒系统治疗强直性脊柱炎合并腰椎骨折患者临床疗效较好,可提高固定效果,骨折部位复位良好,临床应用价值高,值得推广应用.Objective:To analyze the clinical effect of posterior pedicle screw system in the treatment of ankylosing spondylitis with lumbar fractures.Methods:A total of 32 patients with ankylosing spondylitis and lumbar vertebrae fractures from January 20 to June 2019 in Zhujiang Hospital of Southern Medical University were randomly divided into observation group and treatment group(16 cases in each group).The observation group was given traditional vertebrae.The patients underwent internal fixation with pedicle screw fixation.The treatment group was treated with posterior pedicle screw system.The anterior and posterior edge height,spinal canal cross-section,Cobb s angle and spinal nerve injury were observed before and after treatment.Results:After treatment,the cross-sectional area of h the spinal canal,the Cobb^angle and the height of the anterior and posterior margin of the vertebral body were improved,and the improvement of the treatment group was better than that of the observation group.The difference was statistically significant(P<0.05).Spinal nerve injury was improved,and there was no significant difference between the differences(P>0.05).Conclusion:The posterior pedicle screw system was effective in treating ankylosing spondylitis with lumbar vertebrae fractures.It could improve the fixation effect,the frac ture site was well restored,and the clinical application value was high.It was worthy of popularization and application.
