指背筋膜蒂皮瓣与腹部带蒂皮瓣在手指皮肤软组织缺损患者修复中的应用效果

摘　　要：目的:探讨指背筋膜蒂皮瓣与腹部带蒂皮瓣在手指皮肤软组织缺损患者修复中的应用效果。方法:回顾性分析手指皮肤软组织缺损患者88例,根据自愿原则结合病情制定手术方案,采用指背筋膜蒂皮瓣修复的44例为指背组,采用腹部带蒂皮瓣修复的44例为腹部组。比较2组手术相关指标(手术时间、住院时间、术后恢复时间)、预后效果、手指功能评分、并发症情况。结果:2组比较,指背组手术时间[(61.20±12.12)vs(51.21±10.36) min]长于腹部组,住院、术后恢复时间[(6.74±1.10)vs(13.25±2.58)、(3.10±0.52)vs(3.68±0.43) d]均短于腹部组(P<0.05);指背组优良率93.18%高于腹部组的72.73%(P<0.05);术后6个月指背组关节活动评分[(94.25±5.11)vs(86.20±4.54)分]、手指感觉评分[(93.10±4.58)vs(81.23±5.36)分]、外形美观评分[(97.12±2.33)vs(80.55±4.53)分]高于腹部组(P<0.05);指背组并发症发生率6.82%低于腹部组的22.73%(P<0.05)。结论:指背筋膜蒂皮瓣具有术后恢复快、手指功能恢复好、并发症少等优势,是修复手指皮肤软组织缺损的安全有效方案。Objective:To investigate the application effect of dorsal digital fascia pedicled flap and abdominal pedicled flap in the repair of finger skin and soft tissue defects.Methods:A retrospective analysis was performed on 88 patients with finger skin and soft tissue defects,44 cases were repaired with dorsal fascial pedicle flap as the dorsal finger group,and 44 cases were repaired with abdominal pedicled flap as the abdominal group.The surgical indicators(operation time,length of hospital stay,postoperative recovery time),prognostic effect,finger function score,and complications were compared between the two groups.Results:The operation time of the dorsal group[(61.20±12.12)vs(51.21±10.36)min]was longer than that of the abdominal group while the hospitalization and postoperative recovery time[(6.74±1.10)vs(13.25±2.58),(3.10±0.52)vs(3.68±0.43)d]were shorter than the abdominal group(P<0.05);the excellent rate of the dorsal finger group was 93.18%,which was higher than that of the abdominal group 72.73%(P<0.05);6 months after operation,the joint activity score[(94.25±5.11)vs(86.20±4.54)points],finger sensation score[(93.10±4.58)vs(81.23±5.36)points],and aesthetic appearance[(97.12±2.33)vs(80.55±4.53)points]of the dorsal finger group were higher than those in the abdominal group(P<0.05);the incidence of complications in the dorsal finger group was 6.82%,lower than that in the abdominal group 22.73%(P<0.05).Conclusion:Compared with the abdominal pedicled flap,the dorsal fascial pedicled flap has the advantages of quicker postoperative recovery,better finger function recovery,and fewer complications.Therefore,it is a safe and effective solution to repair finger skin and soft tissue defects.
