熏灸法联合象皮生肌膏治疗下肢溃疡的临床护理研究

摘　　要：目的:观察熏灸法联合象皮生肌膏防治下肢溃疡的临床疗效,探讨熏灸法"灸时-疗效"之间的关系和特点,为进一步完善熏灸法的临床护理操作规范提供依据。方法:将80例下肢溃疡患者随机分为对照组、治疗A组、治疗B组和治疗C组,每组20例。对照组创面给予基础护理和象皮生肌膏换药治疗,治疗A、B、C组分别给予与对照组相同处理方法的同时配合不同时间(10 min、20 min、30 min)熏灸治疗,比较各组患者的治疗疗效、创面愈合率、创面愈合时间。结果:各组患者总有效率从高至低依次为:治疗B组(95.0%)>治疗C组(90.0%)>治疗A组(85.0%)>对照组(70.0%),治疗A、B、C组总有效率高于对照组,差异均有统计学意义(P<0.05或P<0.01);治疗A组治疗后第28天创面愈合率(81.3%)明显优于对照组,差异有统计学意义(P<0.05);治疗B组治疗第21、28天创面愈合率分别为48.1%、88.0%,明显优于对照组,差异均有统计学意义(P<0.05或P<0.01);治疗C组治疗第21、28天创面愈合率分别为42.0%、83.7%,明显优于对照组,差异均有统计学意义(P<0.05);治疗A、B、C组愈合时间短于对照组,差异均有统计学意义(P<0.05或P<0.01)。结论:熏灸法联合象皮生肌膏防治下肢溃疡疗效显著。熏灸20 min的疗效较好。Objective:To observe the effect of fumigation moxibustion therapy compressing with xiangpi shengji ointment(XSO)on lower limb ulcer,and to explore the relationship and characteristics of"time-effect"by fumigation moxibustion,so as to provide basis for further perfecting the clinical nursing operation standard of fumigation moxibustion therapy.Methods:The 80 patients with lower limb ulcer were randomly divided into control group,treatment group A,treatment group B and treatment group C,20 in each group.Patients were treated with basic nursing and dressing change of XSO in the control group.In addition to the same treatment as the control group,patients were treated were treated with moxibustion at different time(10 min,20 min,30 min)respectively in treatment group A,B and C.The efficacy,wound healing rate and wound healing time were observed and compared among the four groups.Results:The total effective rate of each group was in the order of high to low as follow,treatment group B(95.0%),treatment group C(90.0%),treatment group A(85.0%)and control group(70.0%).The treatment group A,B and C showed higher total effective rate than control group,and the difference was statistically significant(P<0.05 or P<0.01).Treatment group A showed higher wound healing rate(81.3%)than control group at the 28th day after treatment(P<0.05).The wound healing rate of treatment group B was 48.1%and 88.0%respectively at the 21th and 28th day after treatment,and treatment group B showed higher wound healing rate than control group(P<0.05 or P<0.01).The wound healing rate of treatment group C was 42.0%and 83.7%respectively at the 21th and 28th day after treatment,and treatment group C showed higher wound healing rate than control group(P<0.05).The treatment group A,B and C showed shorter healing time than control group(P<0.05 or P<0.01).Conclusion:Fumigation moxibustion therapy compressing with XSO show significant curative effect on lower limb ulcer.Moxibustion at 20 min shows the best efficacy.
