三级综合医院应对新冠肺炎的政策文本量化研究

摘　　要：目的探讨三级综合医院应对新冠肺炎疫情的政策文件特征,为促进三级综合医院完善新发重大传染病的医院管理政策文件提供借鉴和参考。方法以某三级综合医院为例,检索院办公网2020年1月20日至3月30日的新冠肺炎相关政策文件。基于政策工具类型、时间序列和三级综合医院应对新冠肺炎的防控要素构建三维分析框架,深入分析不同时间阶段和防控要素在政策工具使用上的特点。结果共纳入13个行政职能管理部门的72份政策文件,全部编码数量为233条。该三级综合医院十分重视新发重大传染病的应对工作,各政策发布主体协同性较强,使用了较多的权威型政策工具;在启动防疫模式的第一阶段便出台了涵盖各个防控要素的政策文件,第二阶段已形成较为完善的防疫政策文件体系。在保持一定医疗业务工作量的同时,实现了院内新冠肺炎“零感染”。结论该三级综合医院出台的相关政策可以有效应对新冠肺炎疫情。Objective To explore COVID-19 policy documents in a third class general hospital,and to provide references for promoting the improvement of new hospital management policy documents for major infectious diseases.Methods Taking a third class general hospital as an example,the relevant policy documents of the office network of the hospital from January 20,2020 to March 30 were searched.COVID-19 prevention and control factors in a a third class general hospital are constructed based on policy tools,time series and epidemic prevention and control elements.The three-dimensional analysis framework is constructed to analyze the characteristics of the time stage and different prevention and control factors in the use of policy tools.Results The 72 policy documents of 13 administrative departments were included,with 233 codes.A third class general hospital attached great importance to new major infectious diseases,the main body of policy release had strong coordination,and more authoritative policy tools had been used;in the first stage of launching the epidemic prevention model,policy documents covering various prevention and control elements had been issued,and in the second stage,a relatively complete epidemic prevention policy document system has been formed.At the same time of maintaining a certain amount of medical workload,“zero infection”in a third class general hospital was realized.Conclusion The COVID-19 epidemic situation can be effectively dealt with by the relevant policies promulgated by a a third class general hospital.
