摘　　要：目的探讨胎盘组织中的葡萄糖转运蛋白4(GLUT4)、视黄醇结合蛋白4(RBP4)表达与妊娠糖尿病(GDM)的关系。方法采用前瞻性研究方法,选取2015年1月至2019年11月安徽理工大学第一附属医院淮南市第一人民医院产科就诊的GDM患者100例(GDM组)、正常妊娠分娩的产妇100例(对照组),采用Western blotting法检测两组胎盘组织中的GLUT4、RBP4表达情况,并采用Pearson相关法分析GLUT4、RBP4表达强度与GDM妇女胰岛素抵抗程度的相关性。结果GDM组的空腹血糖(FPG)(5.82±0.55)mmol/L、餐后2h血糖(2hPG)(8.84±1.24)mmol/L、空腹胰岛素(Fins)(16.83±2.67)mIU/L、胰岛抵抗指数(HOMA-IR)(3.97±1.26)水平均高于对照组[FPG(4.81±0.48)mmol/L、2hPG(6.38±1.10)mmol/L、Fins(11.48±1.81)mIU/L、HOMA-IR(2.04±0.85)],差异具有统计学意义(P<0.05);GDM组的GLUT4蛋白表达强度(1.228±0.297)、RBP4蛋白表达强度(1.417±0.306)高于对照组(0.771±0.203)、(0.904±0.168),差异具有统计学意义(P<0.05);GDM组胎盘组织中GLUT4、RBP4蛋白表达强度与HOMA-IR、FPG均呈显著的正相关关系(r值分别为0.663、0.592、0.624、0.606,P<0.05)。结论GDM患者胎盘组织中的GLUT4蛋白、RBP4蛋白表达强度增高,并且与胰岛素抵抗具有一定的相关性。Objective To investigate the relationship between GLUT4 and RBP4 expression and GDM in placental tissue.Methods A prospective study method was used to select 100 cases of GDM(GDM group)patients and 100 cases of normal pregnancy and delivery patients(control group)from Department of Gynecology in the First Affiliated Hospital of Anhui University of Science and Technology from January 2015 to November 2019.Western blotting was used to detect the expression of GLUT4 and RBP4 in the placental tissues of the two groups,and the correlation between the expression intensity of GLUT4 and RBP4 and the insulin resistance of GDM women was analyzed using the Pearson correlation method.Results Fasting blood glucose(FPG)(5.82±0.55)mmol/L,2 h postprandial blood glucose(2 hPG)(8.84±1.24)mmol/L,fasting insulin(Fins)(16.83±2.67)mIU/L,and islet resistance index(HOMA-IR)(3.97±1.26)in GDM group were higher than those in the control group[FPG(4.81±0.48)mmol/L,2 hPG(6.38±1.10)mmol/L,Fins(11.48±1.81)mIU/L,HOMA-IR(2.04±0.85)],the differences were statistically significant(P<0.05).The expression intensity of GLUT4 protein(1.228±0.297)and RBP4 protein(1.417±0.306)in the GDM group were higher than those in the control group(0.771±0.203)and(0.904±0.168),and the differences were statistically significant(P<0.05).The expression intensity of GLUT4 and RBP4 proteins in placental tissue of GDM group had significant positive correlations with HOMA-IR and FPG(r=0.663,0.592,0.624,0.606,P<0.05).Conclusion The expression intensity of GLUT4 protein and RBP4 protein in GDM placenta tissue increased,and there was a certain correlation with insulin resistance.
