自拟方联合药物罐刺络放血法对痛风性膝关节炎患者疼痛血清炎症因子及肝肾功能的影响

摘　　要：目的探讨自拟中药方联合药物罐刺络放血法对痛风性膝关节炎(GKA)患者疼痛、血清炎症因子及肝肾功能的影响。方法在常规治疗基础上,采用中药自拟方联合药物罐刺络放血法,对符合纳入标准的57例GKA患者进行治疗。对比治疗前后患者关节疼痛、中医证候积分、血清炎症因子[肿瘤坏死因子(TNF)-α、白细胞介素(IL)-1β、IL-10]、痛风相关指标[红细胞沉降率(ESR)、血尿酸]、肝肾功能指标[天冬氨酸转氨酸(AST)、丙氨酸转氨酶(ALT)、肌酐(Cr)、尿素氮(BUN)]及膝关节功能评分变化情况。结果与治疗前比,患者治疗后关节疼痛和中医证候积分均明显降低(P<0.01),血清炎症因子TNF-α、IL-1β、IL-10水平下降(P<0.01),痛风相关指标ESR、血尿酸水平明显下降(P<0.01),AST、ALT水平无明显变化(P>0.05),Cr、BUN水平明显下降(P<0.01),Lysholm-Ⅱ量表评分明显提高(P<0.01)。结论常规饮食活动规范基础上,选用自拟中药方联合药物罐刺络放血法治疗GKA,可有效缓解膝关节疼痛,改善患者中医证候和膝关节功能,减轻炎症反应,促进肾功能恢复,但不影响肝功能。Objective To explore the effects of self-made traditional Chinese medicine(TCM)combined with drug cans and bloodletting on pain,serum inflammatory factors and liver and kidney function in patients with gouty knee arthritis(GKA).Methods On the basis of conventional medicine treatment,57 cases of GKA patients who met the inclusion criteria were treated with self-prepared prescription of combined with drug cans pricking bloodletting method.The scores of joint pain and TCM syndrome scores,serum inflammatory factors(TNF-α,IL-1β,IL-10),gout-related indicators(ESR,blood uric acid),liver and kidney function indicators(AST,ALT,Cr,BUN)and knee function score was compared between before and after treatment.Results Compared with before treatment,the patient′s joint pain and TCM syndrome scores were significantly decreased after treatment(P<0.01).TNF-α,IL-1β,IL-10 levels were significantly decreased(P<0.01),ESR,blood uric acid level was significantly decreased(P<0.01),and there was no significant change in AST,ALT levels(P>0.05),and Cr and BUN levels were significantly decreased(P<0.05),and the Lysholm-Ⅱscale score was significantly increased(P<0.01).Conclusions On the basis of routine use of medicine,the use of self-made TCM combined with drug cans pricking bloodletting method to treat GKA can effectively relieve knee pain,improve TCM syndrome and knee function,and reduce inflammation,promotes the recovery of renal function,but does not affect liver function,and has good safety.
