摘　　要：目的:分析本院临床输血申请单的填写情况,总结问题并制定相应改进措施,促进临床用血管理更合理安全。方法:按照卫生部《临床输血技术规范》的填写要求,以项目填写规范、完整为合格,对12667份输血申请单进行逐份检查,不合格的申请单进行分类登记并分析。结果:2016年度共有临床输血申请单6576份,其中不合格的有152份,占比2.3%;2017年度共有输血申请单6091份,不合格的有47份,占比0.77%,相比于2016年度有所下降;两年度不合格的项目中,以医生未签名和输血前九项检查最为常见。结论:分析本院两个年度的输血申请单,2017年度相比于2016年度不合格率有所下降,主要得益于职能部门和输血科的不间断培训和督导检查,以及我院信息化管理的大力推进和使用,但仍存在不规范问题,需建立安全有效的用血保障体系,使输血管理更加科学合理。Objective:To analyze the filling of the clinical blood transfusion application form in the hospital,to summarize the problems and develop corresponding improvement measures,thus to promote the rational and safe management of clinical use of blood.Method:A total of 12667 blood transfusion application forms were checked one by one in accordance with the requirements of Specification for Clinical Transfusion Technology issued by the Ministry of Health.With the completeness and expressiveness filling being the standard,the unqualified application forms were classified,registered and analyzed.Result:In 2016,there were 6576 clinical blood transfusion application forms,among which 152 were unqualified,accounting for2.3%.In 2017,there were 6091 blood transfusion application forms and 47 were unqualified,accounting for 0.77%.Conclusion:Through the analysis of the two years’blood transfusion application forms,it could be seen that the non-qualified rate decreased in 2017 compared with that in 2016.It was mainly due to the continuous training for the blood transfusion departments and supervision from functional departments,as well as the vigorous promotion and use of information management in our hospital.However,there were still some non-standard problems and a safe and effective blood supply system should be established to make blood transfusion management more scientific and reasonable.
