摘　　要：目的比较定向软通道微创术与开颅手术治疗老年高血压脑出血的临床疗效。方法选取2012-01-2019-01中国人民解放军第72集团军医院收治的125例患者为研究对象,分别采取定向软通道微创术(观察组)和开颅血肿清除术(对照组),比较2组术后并发症发生率及预后。结果观察组术后肺部感染、水电解质平衡紊乱、营养不良等并发症发生率明显低于对照组(P<0.05),术后2组颅内再出血及颅内感染率比较差异无统计学意义(P>0.05)。观察组预后良好率不良率均显著优于对照组(P<0.05),2组致残率比较差异无统计学意义(P>0.05)。结论定向软通道微创术可迅速缓解颅内压,术后并发症少且预后良好,特别适用于伴有基础疾病难以耐受全身麻醉的老年患者。Objective To compare the clinical effect between directional soft channel minimally invasive surgery and Craniotomy in treating elderly patients with hypertensive intracerebral hemorrhage(HICH).Methods From January 2012 to January 2019,125 cases admitted to the 72nd Army Hospital of PLA were selected for study.They were given separately directional soft channel minimally invasive surgery(observation group)and Craniotomy(control group).The postoperative complication and prognosis were compared of 2 group.Results The postoperative complication rates include pulmonary infection,water-electrolyte balance disorder,malnutrition in observation group were lower than that of control group(P<0.05),No significant differences on the postoperative intracranial rehaemorrhagia rate and intracranial infection rate were observed of 2 group(P>0.05).The good prognosis rate and the poor prognosis rate in observation group were better than that of control group(P<0.05),There were no significant differences on the disability rate of 2 group(P>0.05).Conclusion The directional soft channel minimally invasive surgery has the advantage of rapid lower intracranial pressure,fewer complications and better prognosis.It can be used particularly for elderly patients with underling health problems,who can't tolerate general anesthesia.
