体外冲击波联合星状神经节超激光照射治疗颈源性头痛疗效观察

摘　　要：目的观察体外冲击波联合星状神经节超激光照射治疗颈源性头痛的临床疗效。方法100例颈源性头痛患者随机分为治疗组和对照组各50例。治疗组给予体外冲击波联合星状神经节超激光照射治疗,对照组予以颈2椎旁神经阻滞治疗。于治疗前、后及治疗后3个月采用视觉模拟评分法(VAS),颈部功能不良指数(NDI),临床疗效愈显率及3个月后的复发率对2组结果进行评定分析。结果治疗后,2组VAS评分、愈显率及NDI指数均较前明显下降(P<0.05);2组VAS评分、愈显率及NDI指数比较差异无统计学意义(P>0.05);3个月后随访,治疗组VAS评分、NDI指数及复发率显著低于对照组,而愈显率显著高于对照组,差异有统计学意义(P<0.05)。结论体外冲击波联合星状神经节超激光照射治疗颈源性头痛近期疗效确切,远期效果优于神经阻滞,复发率低,且免于穿刺治疗的痛苦和风险。Objective To investigate the effect of extracorporeal shock wave and super-laser exposure on stellate ganglion in the treatment of cervicogenic headache.Methods 100 cervicogenic headache patients were randomly divided into the experimental group(n=50 cases)and the control group(n=50 cases)according to the random number table.The experiment group was given extracorporeal shock wave and super-laser exposure on stellate ganglion treatment,while the control group was given C2 nerve block therapy only.VAS scores,NDI scores,the curative and response rates and recurrence rates 3 months later were used to evaluate the functional changes.Results All measurements in both groups were significantly improved than those before training(P<0.05),and there was no statistically significant difference in two groups(P>0.05).The VAS scores,NDI scores,the recurrence rates of the experimental group were lower than that of the control group but higher in the curative and response rates(P<0.05)during the follow-up visit after 3 months.Conclusion The cervicogenic headache patients treated with extracorporeal shock wave and super-laser exposure on stellate ganglion,can not only the short-term effect is obvious,but also the long-term effect is definite,superior to nerve block therapy,free from the risk and pain of puncture therapy.
