血管搭桥联合贴敷术治疗烟雾病的疗效分析

摘　　要：目的探讨血管搭桥联合贴敷术治疗烟雾病的效果。方法选取2016年1月至2018年1月我院收治的84例烟雾病患者作为研究对象,将全部患者按照手术方法分为试验组和对照组,各42例。试验组采用颞浅动脉-大脑中动脉搭桥联合颞肌贴敷术治疗;对照组采用脑-硬膜-颞肌血管贴敷术治疗。比较两组患者的大脑中动脉平均血流速度(Vm)、搏动指数(PI)、生活能力(MRS)评分、再缺血率以及再出血率。结果两组治疗后MRS评分均较治疗前有所降低,且治疗后试验组MRS评分、搏动指数、再缺血率和再出血率均明显低于对照组,平均血流速度要高于对照组,两组比较差异均具有统计学意义(P<0.05)。结论对烟雾病患者采取血管搭桥联合贴敷术治疗,能够明显改善患的平均血流速度和波动指数,提高患者的生活能力,并能明显降低患者术后再缺血和再出血的发生率,具有重要的临床意义。Objective To explore the effect of vascular bypass grafting combined with application in the treatment of moyamoya disease.Methods 84 patients with moyamoya disease admitted to our hospital from January 2016 to January 2018 were divided into the experimental group(n=42)and the control group(n=42)according to different treatment methods.Patients in the experimental group were treated with superficial temporal artery and middle cerebral artery bypass combined with temporal muscle application,and those in the control group were treated with the cerebral-subdural-papillary muscle application.The mean blood flow velocity(Vm),pulsatility index(PI),standard of life(MRS),re-ischemia and rebleeding rate were compared between the two groups.Results The MRS scores of the two groups were lower than those before treatment;the MRS scores,pulsatility index,re-ischemic rate and re-bleeding rate of the experimental group were significantly lower than those of the control group;the average blood flow velocity of the experimental group was higher than that of the control group(P<0.05).Conclusion It is of great clinical significance to treat moyamoya disease with vascular bypass grating combined with application,which can obviously improve the average blood flow velocity and fluctuation index,improve the living ability of patients,and significantly reduce the incidence of reischemia and rebleeding after operation.
