420nm强脉冲光联合治疗寻常痤疮的临床疗效观察

摘　　要：目的观察强脉冲光联合透明质酸修护贴敷料和中药面膜治疗寻常痤疮的临床疗效。方法将146例寻常痤疮患者随机分为三组。实验A组48例,强脉冲光治疗后使用透明质酸修护贴敷料;实验B组49例,强脉冲光治疗后使用中药面膜;对照组49例,强脉冲光治疗后仅用冰袋冰敷。三组患者每周予以1次治疗,3次为1个疗程,连续治疗2个疗程6周,期间第3周、第6周观察并记录皮疹变化情况及不良反应。结果第1个疗程:三组治疗无显著性差异(χ^2=1.432,P>0.05)。6周后,实验A组总有效率为91.7%;实验B组总有效率为89.8%;对照组总有效率为71.4%。三组治疗有显著性差异(χ^2=9.1944,P<0.01);两两比较:实验A组和实验B组间无显著性差异(χ^2=0.0011,P>0.05),实验A组和对照组间有显著差异(χ^2=6.5713,P<0.05),实验B组和对照组之间有显著差异(χ^2=5.2885,P<0.05)。皮肤红斑、瘙痒、水肿和刺痛的不良反应发生率,治疗组都低于对照组。结论联合组治疗寻常痤疮疗效都优于单纯强脉冲光治疗,两联合组之间无显著性差异。强脉冲光联合透明质酸修护贴敷料和中药面膜协同治疗痤疮,治疗的总有效率和满意度比对照组都有明显提高,值得临床推广应用。Objective To observe the clinical efficacy of intense pulsed light combined with hyaluronic acid-containing dressing and traditional Chinese medicine masque in the treatment of acne vulgaris.Methods 146 patients with acne vulgaris were randomly divided into three groups.In experiment group A,48 cases were treated with hyaluronic acid-containing dressing after intense pulsed light treatment.In the experiment group B,49 cases were treated with traditional Chinese medicine masque after intense pulsed light treatment,and 49 cases in the control group,49 cases were treated with only ice packs.The patients in the three groups were treated once a week,three times as a course,for two consecutive courses of 6 weeks,during which(3,6 weeks)the changes of rash and adverse reactions were observed and recorded.Results The first course of treatment:There was no significant difference between the three groups(χ^2=1.432,P>0.05).After 6 weeks,the total effective rate of the experimental group A was 91.7%;the total effective rate of the experimental group B was 89.8%;the total effective rate of the control group was 71.4%.There were significant differences between the three groups(χ^2=9.1944,P<0.01);a pairwise comparison:there was no significant difference between the experimental A group and the experimental B group(χ^2=0.0011,P>0.05),a significant difference between the experimental group A and the control group(χ^2=6.5713,P>0.05),and a significant difference between the experimental group B and the control group(χ^2=5.2885,P<0.05).The incidence of adverse reactions of skin erythema,pruritus,edema and tingling was lower in the treatment group than in the control group.Conclusion The combined groups had better efficacy in treating acne vulgaris than the intense pulsed light treatment alone,and there was no significant difference between the two combined groups.Compared with the control group,the total effective rate and satisfaction of the combined groups are significantly improved,which is worthy of clinical application.
