摘　　要：目的总结40岁以上高龄妇女的妊娠结局,探讨孕期保健对高龄孕妇母婴结局的影响。方法收集2016年1月至2018年12月在郑州大学第一附属医院产科分娩的280例40岁以上产妇的临床资料及妊娠结局,根据产妇的产检情况分为保健组(165例)和未保健组(115例),将两组产妇的资料进行对比分析。结果保健组产妇妊娠期高血压疾病、妊娠糖尿病、胎膜早破、早产及产后出血发生率均低于未保健组,差异有统计学意义(均P<0.05);两组产妇前置胎盘和胎盘植入发生率比较,差异无统计学意义(均P>0.05)。保健组新生儿低出生体质量儿、巨大儿、转入NICU和新生儿出生缺陷发生率均低于未保健组,差异有统计学意义(均P<0.05);两组产妇剖宫产率均比较高,保健组急诊剖宫产率低于未保健组(P<0.05)。结论系统的产前保健、产前筛查和产前诊断对于减少40岁以上孕妇母婴结局的并发症至关重要。Objective To summarize the pregnancy outcome of the elderly women over 40 years old,and to explore the effect of pregnancy health care on the maternal and child outcome of the elderly pregnant women.Methods From January 2016 to December 2018,the clinical data and pregnancy outcome of 280 cases of parturient over 40 years old who gave birth in the First Affiliated Hospital of Zhengzhou University were collected.According to the maternal examination,they were divided into health care group(165 cases)and unhealth care group(115 cases).The data of the two groups were compared and analyzed.Results The incidence of pregnancy-induced hypertension,gestational diabetes mellitus,premature rupture of membranes,premature delivery and postpartum hemorrhage in the prenatal care group were lower(all P<0.05).There were no statistical difference in the incidence of placenta previa and placenta implantation between the two groups(all P>0.05).The incidence of low birth weight,macrosomia,neonatal intensive care unit admissions and birth defects in the prenatal care group were lower(all P<0.05).The incidence of cesarean section was higher in both groups,but the emergency cesarean section in the prenatal care group was lower(P<0.05).Conclusion Systematic prenatal care,prenatal screening and prenatal diagnosis are essential to reduce the complications of the pregnant women over 40 years old and the infants.
