摘　　要：目的探讨功能性鼻内镜手术治疗慢性鼻窦炎的疗效及预后影响因素。方法收集2016年11月至2018年1月在江苏省如皋市人民医院确诊的100例慢性鼻窦炎患者的临床资料,根据治疗方式分为观察组(52例)和对照组(48例)。观察组患者采用功能性鼻内镜手术治疗;对照组患者采用常规手术治疗,比较2组术后疗效、患者生存质量、疼痛程度、术后并发症等指标,采用多元logistic回归影响手术预后的影响因素。结果观察组手术总有效率98.18%高于对照组的总有效率87.50%,观察组总并发症发生率3.84%低于对照组18.75%(P<0.05);2组患者手术后的SNOT-20评分和疼痛程度评分较手术前的评分均有所下降,且观察组的鼻腔鼻窦结内测试-20(SNOT-20)评分和疼痛程度评分降低较对照组多(P<0.05)。伴鼻息肉、治疗依从性和多发病变为影响功能性鼻内镜手术治疗预后的危险因素,其风险比分别为5.63、7.54和2.84倍。结论功能性鼻内镜手术治疗慢性鼻窦炎的疗效显著,伴有鼻息肉、治疗依从性和多发病变为影响预后的危险因素。Objective To explore the efficacy and influencing factors of functional endoscopic sinus in the treatment of chronic sinusitis.Methods Totally 100 cases who were diagnosed chronic sinusitis were collected from Rugao people's Hospital of Jiangsu Province,from November 2016 to January 2018,randomLy divided into the observation group(52 cases)and the control group(48 cases).The observation group was treated with functional nasal endoscopy,while the control group was treated with routine surgery.The therapeutic effect,quality of life,degree of pain and postoperative complications between the two groups were compared,the influencing factors of operative prognosis were analyzed by multiple logistic regression.Results The total effective rate of the observation group(98.18%)was higher than that of the control group(87.50%),the total complication rate of the observation group(3.84%)was lower than that of the control group(18.75%)(P<0.05).The score of post-operative spot-20 and quality of life in the two groups were lower than that before operation,and the score in observation group was lower than that in the control group(P<0.05).The risk factors for the prognosis of functional endoscopic sinus surgery were associated with nasal polyps,treatment compliance and multiple lesions,the risk ratios were 5.63,7.54 and 2.84 times,respectively.Conclusion Functional endoscopic hand The curative effect of the operation was significant,with nasal polyps,compliance and multiple lesions were the risk factors.
