明目汤联合甲钴胺对青光眼视神经损伤患者P-VEP及视野的影响

摘　　要：目的探讨明目汤联合甲钴胺对青光眼视神经损伤患者图形视觉诱发电位(P-VEP)及视野的影响。方法回顾性分析2017年1月至2018年1月昆明市中医医院收治的60例青光眼视神经损伤患者为研究对象,根据治疗方式的不同分为治疗组和对照组,每组30例。青光眼手术后治疗组采用明目汤联合甲钴胺治疗,对照组使用甲钴胺进行治疗,治疗4周。比较2组临床疗效及不良反应发生率、治疗前后图形视觉诱发电位(P-VEP)变化情况、视野变化情况。结果治疗组总有效率为100.00%,高于对照组的73.33%(P<0.05);与治疗前相比,治疗后2组P-VEP振幅均升高,且治疗组显著高于对照组,2组P-VEP潜伏期均缩短,且治疗组显著短于对照组(P<0.05);与治疗前相比,治疗后2组光敏度、视敏度均升高,且治疗组显著高于对照组(P<0.05);2组不良反应率经比较差异无统计学意义(P>0.05)。结论明目汤联合甲钴胺可改善青光眼视神经损伤患者的病情,减轻视神经损害,极大地缩短视力改善时间,疗效显著,且安全性良好。Objective To explore the effect of Mingmu Decoction combined with mecobalamin on P-VEP and visual field in patients with optic nerve injury in glaucoma.Methods A retrospective analysis was performed on 60 patients with glaucomatous optic nerve injury treated in Kunming Traditional Chinese Medicine Hospital from January 2017 to January 2018.The patients were divided into a treatment group and a control group according to different treatment methods,30 cases in each group.After glaucoma surgery,the treatment group was treated with Mingmu Decoction combined with mecobalamin,and the control group was treated with mecobalamin for 4 weeks.The clinical efficacy and incidence of adverse reactions,changes in graphic visual evoked potential(P-VEP),and visual field changes before and after treatment were compared between the two groups.Results The total effective rate in the treatment group was 100.00%,which was higher than 73.33%in the control group(P<0.05).Compared with before treatment,the P-VEP amplitude of the two groups increased after treatment,and the treatment group was significantly higher than that of the control group.The incubation period of P-VEP of the two groups shorted,and the treatment group was significantly shorter than the control group(P<0.05).Compared with before treatment,the photosensitivity and sensitivity of the two groups increased after treatment,and the treatment group was significantly higher than that of the control group.(P<0.05);the adverse reaction rates in both groups were 6.66%,and there was no significant difference after comparison(P>0.05).Conclusion Mingmu decoction combined with mecobalamin can improve the condition of patients with glaucomatous optic nerve damage,reduce optic nerve damage,greatly shorten the time to improve vision,have significant effects,and have good safety.
