某院糖尿病门诊口服降糖药的使用调查分析与评价

摘　　要：目的通过对某院2018年12月份内分泌专科门诊降糖药处方进行统计分析和评价,了解门诊口服降糖药的使用情况,为临床合理、有效、经济使用降糖药提供参考。方法统计处方中患者性别、年龄的分布情况,各种降糖药的药品名、实际使用总剂量以及联合用药与合理用药情况,采用世界卫生组织推荐的限定日剂量(DDD)、用药频度(DDDs)和药物利用指数(DUI)等方法,分析门诊口服降糖药的使用情况。结果本次调查处方1 586张,其中男性731例(46.09%),女性855例(53.91%)。年龄大于50岁的人数最多,占74.59%。用药频度前3位的是二甲双胍片(格华止)、格列美脲片(亚莫利)以及阿卡波糖片(拜唐苹),药物利用指数排名前3位的药物是二甲双胍肠溶胶囊,西格列汀片(捷诺维)以及瑞格列奈分散片(孚来迪)。联合用药928例,其中二联用药666例,三联用药246例,四联用药16例。结论该院糖尿病患者用药在药物遴选、联合用药及药物经济性等方面基本合理。OBJECTIVE The usage of oral hypoglycemic drugs in endocrinology outpatients of a hospital in December 2018 was investigated,through statistical analysis and evaluation of the prescription of hypoglycemic drugs,to provide a reference for the rational,effective and economic use of hypoglycemic drugs in the clinic.METHODS The use of oral hypoglycemic agents in outpatient clinics was analyzed by using the methods of defined daily dose(DDD),frequency of use(DDDs)and drug use index(DUI)recommended by the world health organization.The distribution of gender and age of patients in the prescription and the drug names of various hypoglycemic drugs in the prescription,the actual total dose,the combination medication and the rational medication were counted.RESULTS There were 731 male patients(46.09%)and 855 female patients(53.91%)in the 1586 oral hypoglycemic drugs prescription and the largest number of people were over 50 years,accounting for 74.59%.According to the order of DDDs,the top three are Metformin tablets,Glimepiride tablets,and Acarbose tablets.The top three drugs in the DUI are Metformin Enteric-Coated Capsules,Sitagliptin Tablets,and Rieglinide dispersible tablets.There were 928 cases of combined drugs,including 666 cases of duplex medication,246 cases of triple medication and 16 cases of quadruple medication.CONCLUSION In terms of drug selection,combined use and drug economics,the application of hypoglycemic drugs in the hospital is basically reasonable.
