摘 要:目的评估太原市无偿献血者艾滋病病毒(HIV)感染情况,血液筛查后经输血传染HIV的残余风险度。方法回顾性分析2013-2017年无偿献血者HIV检测结果,比较初次献血者组和重复献血者组中男性和女性的HIV阳性率,用流行率/窗口期数学模型法对残余危险度进行评估。结果共检测无偿献血者血液标本444501份。初次献血者组男性和女性的抗-HIV阳性率分别为0.73‰和0.13‰,重复献血者组男性和女性的抗-HIV阳性率分别为0.33‰和0.02‰。经评估抗-HIV筛查后总残余风险为14.81×10-6,其中男性无偿献血者HIV残余风险为14.03×10-6,女性为0.78×10-6,初次献血者HIV残余风险为9.70×10-6,重复献血者为5.11×10-6。结论太原市无偿献血者HIV流行率初次献血者高于重复献血者,男性高于女性。现有条件下,传播HIV的残余风险男性献血者较女性大,初次献血者较重复献血者大。Objective To analyze the status of HIV/AIDS infection among voluntary blood donors in Taiyuan, and assess the residual risk of HIV transmission via transfusion after blood screening. Methods The HIV testing results of voluntary blood donors from 2013 to 2017 were analyzed retrospectively. HIV positive rates of male and female blood donors were compared between the initial donors and the repeat donors, and the residual risk was evaluated by prevalence or window period mathematical model. Results A total of 444 501 blood samples from unpaid donors were tested in the five years. The positive rate of HIV was 0.73‰ among male initial blood donors, and 0.13‰ among female initial blood donors;0.33‰ among male repeat blood donors, and 0.02‰ among female repeat blood donors. The total residual risk of HIV infection for voluntary blood donors after anti-HIV screening was 14.81×10-6, of which 14.03×10-6 for male and 0.78×10-6 for female, 9.70×10-6 for initial blood donors and 5.11×10-6 for repeat blood donors. Conclusion The HIV infection rate of initial blood donors is higher than that of repeat blood donors in Taiyuan. The HIV infection rate of male donors is higher than that of female donors. The residual risk of HIV transmission from male donors is greater than that from female donors, and the residual risk of HIV transmission of initial blood donors is greater than that of repeat blood donors.
