摘  要:目的探索极低体重早产儿血清25-羟维生素D(25(OH)D3)及血浆中性粒细胞明胶酶相关脂质运载蛋白(sNGAL)的表达与支气管肺发育不良的关系。方法选取2015年6月至2018年9月于我院接受治疗的128例极低体重早产儿,患儿男性69例,女性59例;平均胎龄(29.64±2.17)周;平均体重(1113.48±115.17)g;为研究对象,按照是否患有支气管肺发育不良将患儿分为两组,106例未患肺部发育不良的患儿为对照组,22例患儿肺部发育不良的患儿为观察组。观察并比较两组患儿的住院时间,对可能影响极低体重早产儿支气管肺发育不良的因素进行多因素Logistic回归分析。结果观察组患儿的呼吸机治疗天数(24.69±6.55)天、氧疗天数(32.49±5.97)天以及住院天数(59.74±11.38)天显著高于对照组,(P<0.05)。两组患儿的性别、剖宫产、出生季节、妊娠期糖尿病、产前应用激素、PDA、医院感染无显著差异,(P>0.05)。观察组患儿的出生体重(1029.85±144.57)g,出生胎龄(28.04±1.58)周、1分钟新生儿评分(5.26±1.22)分、5分钟新生儿评分(7.15±1.58)分以及血清25-羟维生素D水平(32.41±14.66)nmol/L显著低于对照组,(P<0.05)。观察组的中性粒细胞明胶酶相关脂质运载蛋白水平(100.15±19.21)μg/L显著高于对照组,数据比较具有统计学意义(P<0.05)。结果显示,血清25-羟维生素D和中性粒细胞明胶酶相关脂质运载蛋白水平是影响早产儿发生支气管肺发育不良的独立危险因素(P<0.05)。结论支气管肺发育不良显著延长了极低体重早产患儿的治疗时间,增加了患儿的死亡风险,而血清25-羟维生素D和中性粒细胞明胶酶相关脂质运载蛋白水平可能是影响患儿支气管肺发育不良的独立危险因素。Objective To explore the relationship between serum 25-hydroxyvitamin D(25(OH)D3)and plasma neutrophil gelatinase-associated lipocalin(sNGAL)and bronchopulmonary dysplasia in very low birth weight preterm infants Methods 128children with very low birth weight who were treated in our hospital from March 2017to September 2018were enrolled.The children were divided into two groups according to whether they had bronchopulmonary dysplasia.106patients did not develop lung development.The poor children were the control group,and the 22children with pulmonary dysplasia were the observation group.The hospitalization time of the two groups was observed and compared,and multivariate logistic regression analysis was performed on the factors that may affect the bronchopulmonary dysplasia in very low birth weight premature infants.Results The number of ventilator treatment days(24.69±6.55)days,oxygen therapy days(32.49±5.97)days,and hospitalization days(59.74±11.38)days in the observation group were significantly higher than those in the control group.The data were statistically significant(P<0.05).There were no significant differences in gender,cesarean section,birth season,gestational diabetes,prenatal steroids,PDA,and nosocomial infection between the two groups.The data were not statistically significant(P>0.05).The weight of the body in the observation group was(1029.85±144.57)g,the gestational age(28.04±1.58)weeks,the 1-minute neonatal score(5.26±1.22),the 5-minute neonatal score(7.15±1.58),and the serum 25The level of hydroxyvitamin D(32.41±14.66)nmol/L was significantly lower than that of the control group,and the data were statistically significant(P<0.05).The neutrophil gelatinase-associated lipocalin level(100.15±19.21)μg/L in the observation group was significantly higher than that in the control group,and the data were statistically significant(P<0.05).The results showed that serum 25-hydroxyvitamin D and neutrophil gelatinase-associated lipocalin levels were independent risk factors for bronchopulmonary
